ICU谵妄患者发病因素及治疗对策

目的:探讨ICU患者谵妄的发生率和发病因素,并拟定相关治疗策略。方法:总结谵妄患者病例资料627例,以CAM-ICU为评判谵妄标准,分析谵妄的发病因素及治疗措施。结果:627例患者中发生谵妄201例(32.05%),谵妄病例中感染所致48例(23.88%),代谢异常所致40例(19.90%),缺氧所致36例(17.91%),手术及创伤所致31例(15.42%),药物不良反应所致22例(10.95%),环境因素所致14例(6.97%),难以确定原因10例(4.97%)。结论:ICU患者谵妄发生率较高,发病因素以感染、代谢异常、缺氧及手术创伤为主,治疗应积极消除病因,加强护理和医患沟通。

Pathogenic factors and therapy for ICU patients with delirium

Objective To study the incidence rate and pathogenic factors of delirium on ICU paitents,and to sketch corresponding therapies.Method The pathogenic factors and therapies were analyzed based on 627 samples accumulated from January 1st of 2011 to December 31st of 2011,taking CAM-ICU as the standard for judging delirium.Results Among the 627 samples,delirium occured on 201 patients(32.05%),among which 48 were caused by infection(23.88%),40 were caused by metabolic disorder(19.90%),36 were caused by anoxia(17.91%),31 were caused by operation and injury(15.42%),22 were caused by adverse reaction of medication(10.95%),14 were caused by environmental factors(6.97%),and 10 were caused by uncertain factors(4.97%).Conclusion The incidence rate of delirium on ICU patients is high,mainly caused by infection,metabolic disorder,anoxia,and operational injury,whereby the therapy is to activelly eliminate the pathogenic factors,enhance the nursing and communicate between the patients and doctors.
