过滤去除白细胞过程中的红细胞溶血

摘    要:目的 探讨过滤去除白细胞过程中红细胞溶血的原因 ,寻找解决方案。方法 采用不同处理模式 :先过滤后离心 /先离心后过滤、用 /不用生理盐水预浸润滤器制备红细胞悬液。 4 8u全血 ,其中 30u全血 ,每单位分为 3份 (A、B、C袋 )。按A、B、C袋将 30u全血分为 3组 ,并分别给予不同处理。A组为对照组不过滤 ,B组为先离心后过滤组 ,C组为先过滤后离心组。B ,C两组过滤前均未用生理盐水浸润滤器。另外 1 8u全血在过滤前用生理盐水浸润滤器 ,然后采用先过滤后离心模式处理。测定各组RBCs的游离血红蛋白和K+ 浓度。结果 在不用生理盐水预浸润滤器的条件下 ,过滤后的红细胞悬液Hb和K+ 的浓度明显高于对照组 (P <0 .0 5 ) ,其中先过滤后离心模式制备的红细胞悬液Hb和K+ 的浓度明显高于先离心后过滤的红细胞悬液 (P <0 .0 5 ) ,采用生理盐水预浸润处理后过滤的红细胞悬液Hb和K+ 的浓度与对照组无显著差异 (P >0 .0 5 )。结论 现用的滤器过滤去除白细胞能引起一定程度的红细胞溶血 ,先过滤后离心模式较先离心后过滤模式更容易导致红细胞溶血 ,预先用生理盐水浸润滤盘 ,能有效地减少过滤引起的溶血

Abstract:Objective To explore the causes of hemolysis during the removal of white blood cell (WBC) and to approach the protection of red blood cell(RBC) from hemolysis.Methods The four treatment methods were introduced as following:filtration before centrifugation and centrifugation before filtration,with/without soaking the filter using physiological saline before filtration.Fourty eight units of whole blood were used in this study.Thirty units were treated without soaking the filter by physiological saline before filtration,while the rest 18 units were treated with soaking filter by physiological saline.Of the first 30 units,every unit was divided into 3 parts(A,B and C).Part A was the control,without being filtrated.Part B was centrifuged before filtration.Part C was filtrated before centrifugation.The rest 18 units were first filtrated then centrifuged.At the end,free Hb and K + concentrations of RBCs of all 48 units were measured respectively.Results Free Hb and K + concentrations of Part B and Part C were both significantly higher than that of Part A ( P <0.05),and that of C group was significantly greater than B group ( P <0.05),that of the filter soaking by physiological saline did not significantly increase hemolysis,compared to Part A ( P >0.05).Conclusion Removal of WBC with current filter can result in some degree in hemolysis.It has a higher possibility to cause hemolysis to perform filtration before centrifugation than to perform filtration after centrifugation during preparation of blood.Soaking the filter with physiological saline prior to filtration can effectively reduce hemolysis induced by filtration.
