21种HPV亚型检测在宫颈病变诊断及预测中的价值

摘    要:目的探讨HPV亚型感染与宫颈病变的关系。方法360例宫颈病变患者组织病理学检查诊断为宫颈炎/宫颈尖锐湿疣254例,宫颈上皮内瘤变(CIN)52例(其中CINⅠ12例、CINⅡ16例、CINⅢ24例),宫颈癌54例,采用导流杂交基因芯片技术检测宫颈脱落细胞中HPV亚型,分析不同病变的HPV感染情况。结果本组HPV感染率为83.3%(300/360);宫颈炎/宫颈尖锐湿疣、CIN、宫颈癌高危型感染率明显高于低危型(P〈0.01);高危型HPV感染率随宫颈病变程度加重而升高;不同宫颈病变中单一型感染率明显高于多重型感染率(P〈0.01),多重型感染率随宫颈病变程度加重而升高。不同HPV亚型致病性不同。结论HPV亚型检测可用于宫颈病变的筛查病情判断。

Abstract:Objective To investigate the relation between different subtypes of HPV and cervical lesions. Methods Three hundreds and sixty patients with cervical lesions were diagnosed by pathological examination 254 cases of cervicitis/ cervical condyloma acuminate,52 cases of cervical intraepithelial neoplasia (CIN, 12 cases of CIN Ⅰ ,16 cases of CIN Ⅰ , 24 cases of CINⅢ ) ,52 cases of cervical cancer, the HPV subtypes were detected by flow-through hybridization gene chip technology, the HPV infection of different lesions were analyzed. Results The HPV infection rate of this group was 83.3% (300/360) ;The rates of high-risk HPV infection were significantly higher than that of low-risk HPV infection in cervicitis / cervical condyloma acuminate, CIN and cervical cancer ( P 〈 0.01 ) ; The rates of high-risk HPV infection increased with the degree of cervical lesions ; The rates of single HPV subtype infection were significantly higher than that of multiple HPV subtypes infection in different cervical lesions (P 〈0.01 ), the rates of multiple HPV subtypes infection increased with the degree of cervical lesions. The pathogenicity of different HPV subtypes was different. Conclusion The detection of HPV subtypes can be used in the screening of cervical lesions and predicting of progression.
