肝衰竭纤维化指标的动态观察和预后评估

目的探讨病毒性肝炎肝衰竭患者血清肝纤维化指标的动态变化。方法选择本院50例肝衰竭患者（肝衰竭组），与慢性肝炎（30例），正常体检者（30）例，进行血清肝纤维化指标比较，并动态观察肝衰竭患者血清肝纤维化指标。结果与正常对照组比较，慢性肝炎组、肝硬化组两组的四种血清肝纤维化指标HA、LN、CⅣ和PCⅢ均显著高于对照组，慢性肝炎组与肝硬化组两组间四种血清肝纤维化指标比较，肝硬化组HA显著高于慢性肝炎组，LN、CⅣ和PCⅢ三个指标两组间差异均无统计学意义。入院时患者肝纤维化指标均比较高，治疗2周、1个月、3个月后，总共死亡25例，HA、LN水平均下降。但CⅣ、PCⅢ变化不明显。结论随着病情的进展或好转，患者肝纤维化指标可反映肝纤维化的程度，可对预后进行评估，早期进行肝纤维化的治疗，将有望逆转。

Dynamic Observation on Fibrosis Indices and Prognosis Assessment of Hepatic Failure

Objective To explor the dynamic changes of liver fibrosis.indexes in patients with liver failure. Methods Fifty cases patients with liver failure in our hospital, and chronic hepatitis （30 cases）, normal subjects （30cases） were selected,Indexes of the serum hepatic fibrosis was compared. Results Compared with normal control group,the serum HA,LN,C Ⅳ d PC Ⅲ of chronic hepatitis,cirrhosis liver fibrosis were significantly higher. Compared with chronic hepatitis,above indexes of liver fibrosis group was significantly higher,there was no significant difference between the two groups in indexes. LN,CⅣ and PC Ⅲ. Treatment 2 weeks, 1 month,3 months,a total of 25 patients died,HA,LN levels in serim were decreased. However,C Ⅳ ,PC Ⅲ did not change significantly. Conclusion As the disease progress or improvement in liver fibrosis in patients could reflect the degree of liver fibrosis can be assessed on the prognosis, early treatment of liver fibrosis, is expected to reverse.
