硝普钠、多巴胺和多巴酚丁胺治疗扩张型心肌病顽固性心力衰竭疗效观察

目的：探讨硝普钠、多巴胺和多巴酚丁胺治疗扩张型心肌病顽固性心力衰竭的临床疗效。方法：32例患者给予休息、吸氧、利尿剂、地高辛、血管紧张素转换酶抑制剂（ACEI）类药物常规处理，存在心律失常，给予相应的抗心律失常治疗，同时给予5％葡萄糖注射液250ml＋硝普钠10mg＋多巴胺40mg＋多巴酚丁胺40mg，以10滴／min静脉滴注，3～5d后症状缓解停用硝普钠，继续使用多巴胺和多巴酚丁胺，疗程15d。结果：32例患者治疗后心功能有显著改善（均P〈0．05）。结论：硝普钠、多巴胺和多巴酚丁胺治疗扩张型心肌病顽固性心力衰竭疗效明显，安全可靠。

Sodium nitroprusside,dopamine with dobutamine in treatment of refractory heart failure of dilated cardiomyopathy

Objective:To explore the clinical effect of sodium nitroprusside,dopamine with dobutamine in treatment of refractory heart failure of dilated cardiomyopathy. Methods:32 cases received regular treatment such as rest, oxygen, diuretic, digoxin, ACEI etc. When arrhythmia existed the drugs for anti-arrhythmia was given. Meanwhile sodium nitroprusside,dopamine with dobutamine were given too. After 3~5 days of symptom relieved, sodium nitroprusside was stoped, dopamine and dobutamine continued, the whole treatment course is 15 days. Results: The heart function in 32 cases improved obviously after treatment(P<0.05). Conclusion: The curative effect of sodium nitroprusside, dopamine with dobutamine in treatment of refractory heart failure of dilated cardiomyopathy is obvious, safety and credibility.
