医院感染铜绿假单胞菌耐药谱及质粒图谱分析

摘    要:目的了解医院感染铜绿假单胞菌不同血清型的耐药谱及分子流行病学特性。方法耐药谱选用K—B法和MIC、Alkalinelysis小量快速提取质粒,0.7%琼脂糖凝胶电泳获得质粒DNA图谱。结果呼吸病区患者和氧气湿化瓶分离的PA血清4型均耐AP、CZ、T和NOR,烧伤科患者PA血清6型分别耐AP、CZ、T和AP、CB、CZ、CXT、T,外科患者血清11型均耐AP、CB、Pip、CZ、CFP、CXT、CAZ、T、AN、GM和TM11种抗生素,4株对照菌不同。质粒图谱显示:呼吸病区患者和氧气湿化瓶分离菌均有约4.0、1.8、1.7、Mdal3条质粒带;烧伤患者含有4.0、2.0、1.8Mdal3条和4.1Mdal1条质粒带;外科患者分离菌均有1条约3.9Mdal的质粒带;对照菌不同。结论采用联合分型技术对医院不同病区患者分离的不同血清型PA其同源性不同。治疗感染选用抗生素种类也不同,为临床医生在经验用药的基础上选用敏感有效抗生素起指导作用,并为医疗器械消毒效果观察提供依据。

Abstract:OBJECTIVE In order to know the different drug-fast spectrums of hospital infection Pseudomonas aeruginosa with different types of blood serum and the characteristic of molecular epidemiology were studied. METHODS the K-B manner and MIC are used to obtain drug-fast spectrum and the alkaline lysis is to extract small quantity of plasmid is got from cataphoresis of 0.7% agarose congelation. RESULTS Respiratory patients with type 4 PA blood serum separated from oxygen wet-processed bottle can endure AP, CZ, T and NOR, and to burned patients with type 6 PA blood serum can bear AP, CZ, Tand AP, CB, CZ, CXT, T respectively, moreover, surgical patients with type 11 blood serum will stand 11 kinds of antibiotics like AP, CB, Pip, CZ, CFP, CXT, CAZ, T, AN, GM, as well as TM, with 4 different control bacteriam, illustration of plasmid diagral spectrum: both respiratory patients and the separated bacteria out of oxygen wet-processed bottle contain 3 plasmid bands of 4.0, 1.9 and 1.7 Mdal, and 2 burned patients show 4.0, 2.0 and 1.8 Mdal plasmid bands and 4.1 Mdal one, meanwhile, surgical patients and the separated bacteria have 3.9 Mdal plasmid band but with different control bacteria. CONCLUSION By using combined category technique, we find out it quite different for the common source when separating a variety of PA blood serum from diversed patients in the same hospital, so that selection of infection, which will direct clinic doctors to correctly choose sensitive and effective antibiotics upon their past experience and simultaneously provide essential basis for supervising sterilised effect of medical apparatus and instrument.
