合并心力衰竭孕妇剖宫产术中循证护理应用效果研究

目的探讨妊娠合并心力衰竭(简称心衰)孕妇剖宫产术中循证护理应用效果。方法将100例行剖宫产的合并心衰孕妇随机均分为对照组与观察组,对照组行常规护理,观察组行循证护理,比较护理前后血清心肌酶谱、脑钠肽(BNP)水平及视觉模拟评分(VAS评分)、新生儿Apgar评分、并发症发生率、护理满意率、心理状况及健康行为养成等的差异。结果对照组护理前后血清心肌酶谱与BNP水平比较差异无统计学意义(P>0.05),观察组护理前后比较差异有统计学意义(P<0.01),且与对照组护理后比较差异有统计学意义(P<0.05);护理后VAS评分、新生儿Apgar评分、并发症发生率及满意率组间比较差异有统计学意义(P<0.05);观察组并发症发生率低于对照组(P<0.05);观察组满意率高于对照组(P<0.05);所有孕妇护理前后心理状况均有明显改善(P<0.05),且观察组护理后心理状况优于对照组(P<0.05);观察组健康行为养成情况优于对照组。结论循证护理能够明显改善行剖宫产术的合并心衰产妇及婴儿生活质量,有助于促进健康行为的养成。

Research on efficacy of evidence-based nursing among pregnant women with heart failure underwent cesarean section surgery

Objective To investigate the efficacy of evidence-based nursing（EBN） among pregnant women with heart failure underwent cesarean section surgery.Methods 100 cases of pregnant women,complicated with heart failure and receiving cesarean section surgery,were randomly divided into control group and observation group,50 cases for each group.Conventional care and EBN were performed in control group and observation group.Serum levels of cardiac muscle enzyme spectrum and brain natriuretic peptide（BNP）,visual simulation scale（VAS） score,Apgar score,incidence of complications,nursing satisfaction,psychological status,health behaviors development and so on were compared between the two groups.Results There was no statistical difference of serum muscle enzymes indicators and BNP level in control group before and after care（P0.05）,and there existed statistical difference in observation group（P0.05）.There were some statistical differences of VAS score,Apgar score,incidence of complications and satisfaction between the two groups（P0.05）.The satisfaction rate of control group was lower than that of observation group（P0.05）.The two groups′ mental health was significantly improved after care（P0.05）,and there was statistical difference of the psychological conditions between the two groups（P0.05）.The healthy behaviors were better in observation group.Conclusion ENB could significantly improve the quality of life among pregnant women,complicated with heart failure and receiving cesarean section surgery,and infants,and could promote the cultivation of healthy behaviors.
