糖化血红蛋白和尿转铁蛋白联合检测在糖尿病肾病早期诊断中的临床价值

摘    要:目的探讨糖化血红蛋白A1(HbA1c)和尿转铁蛋白(UTRF)联合检测对糖尿病肾病(DN)的早期诊断价值及其与尿微量清蛋白(mAlb)的相关性。方法对128例糖尿病(DM)患者和50例健康者进行HbA1c、UTRF和mAlb检测和比较。结果 DN患者HbA1c、UTRF浓度显著高于健康者(P<0.05),HbA1c、UTRF均与mAlb呈正相关,相关系数分别为0.960和0.954(P<0.05);HbA1c、UTRF联合检测将检测阳性率提高至80.7%。结论 HbA1c、UTRF联合检测是反映DM早期肾损伤的灵敏、可靠指标,对DN早期诊断具有重要意义。

Abstract:Objective To explore the diagnostic significance of glycosylated hemoglobin A1(HbA1c) and urinary transferring(UTRF) in early diabetic nephropathy and to investigate the correlation beteen HbA1c,UTRF and urinary microalbumin(mAlb).Methods HbA1c,UTRF and mAlb were mearsured in 128 diabetes patients and 50 healthy subjects,and the results were compared.Results The concentrations of HbA1c and UTRF in diabetic nephropathy peatints were significantly higher than those in healthy subjects(P0.05),HbA1c and UTRF level was significantly positive correlated with mAlb(r=0.960 or 0.954,P0.05).Combined positiveness of HbA1c and UTRF in early diabetic nephropathy was 80.7%.Conclusion HbA1c and UTRF could be sensitive and reliable for detection of early renal impairment in diabetic patients,and might be with great importance in early diagnosis of diabetic nephropathy.
