组织多普勒Tei指数评价冠心病患者的左心功能

目的组织多普勒Tei指数评价冠心病患者左心的功能,探讨其临床应用价值。方法选择经冠状动脉造影证实前间壁Ⅱ组和下侧壁Ⅲ组心肌梗死患者35例作为病例组,另选25例正常人Ⅰ组作为对照组。TDI成像条件下,于心尖四腔心和心尖二腔心切面获得前壁、间隔、左室侧壁和下壁的二尖瓣环频谱曲线,分别计算出4个位点的Tei指数。结果（1）TDI测量Tei指数结果比较：Ⅱ组Tei指数0.58±0.19和Ⅲ组0.60±0.21分别与Ⅰ组0.39±0.07比较,Tei指数增加,有明显统计学意义（P〈0.01）。（2）前间壁心梗组中,测得前壁和间隔的Tei指数高于侧壁和下壁（0.615和0.612,0.584和0.554;P〈0.05）;下侧壁心梗组中,测得下壁和侧壁的Tei指数高于前壁和间隔（0.615和0.596,0.561和0.56;P〈0.05）。结论TDI-Tei指数不但能清晰显示频谱,提高测值精确性,而且能分析不同病变室壁的整体功能等优点。

Value of Measuring Tei Index by Tissue Doppler Imaging in Coronary Atherosesclerotic Heart Disease

Objective Tei index is a simple and reproducible method for the assessment of overall cardiac function.In this study,we evaluated the applicability and usefulness of TDI as a tool to calculate Tei index.Methods Thirty-five patients with previous myocardial infarction and twenty-five health subjects were included.The subjects were divided into three groups: Ⅰ group(health),Ⅱ group(anterior-MI),Ⅲ group(inferior-MI).Measuring method: TDI of mitral annulus at four sites corresponding to LV eroseptal、 lateral、 anterior and inferior walls at apical four-chamber view and two-chamber view.The time from the end of mitral filling flow velocity to the next beginning of it(b) and the time of aortic ejection(a) were measured and Tei index was calculated from equation Tei index=b-a/a.Results(1)Tei index significantly increased incomparison with those in the heathy age-matched subjects: previous MI(Ⅱ group Tei index 0.58±0.19;Ⅲ group Tei index 0.60±0.21;Ⅰ group Tei index 0.39±0.07).(2)In patients with Ⅱ group,the rise in Tei index by TDI values was found to be more pronounced at the anterior and septal sites of the mitral annulus(0.615 and 0.612,0.584 and 0.554;P<0.05).The patients with Ⅲ group,the rise in Tei index by TDI values was found to be more pronounced at the inferior and septal sites of the mitral annulus(0.615 and 0.596,0.561 and 0.56;P<0.05).
