摘    要:目的:探讨分子标志物在高级别胶质瘤患者预后评估中的价值。方法:收集2010年09月至2015年07月之间经我院收治的高级别胶质瘤患者31例,免疫组织化学染色检测患者肿瘤组织中Ki-67、6-氧-甲基鸟嘌呤-DNA甲基转移酶(MGMT)、表皮生长因子受体(EGFR)、p53和基质金属蛋白酶-9(MMP-9)的表达;电话及门诊随访患者生存状况,分析上述分子标志物在高级别胶质瘤患者预后评估中的价值。结果:Ki-67和MGMT高表达患者的PFS明显低于低表达患者(P<0.05),但MMP-9、EGFR和p53表达水平与PFS间未见明显相关性(P>0.05)。多因素分析发现Ki-67是影响PFS的独立预后因素(RR=5.19,P<0.05)。结论:Ki-67是影响高级别胶质瘤患者PFS的独立危险因素,在预后评估中可能具有潜在的应用价值。

Abstract:Objective:To evaluate the prognostic value of molecular markers in patients with high-grade glioma.Methods:From September 2010 to July 2015,31 patients with high-grade glioma were collected and confirmed by pathology.Immunohistochemistry was used to detect the expression levels of Ki-67,O6-methylguanine-DNA methyltransferase (MGMT),epidermal growth factor receptor (EGFR),p53 and matrix metalloproteinase-9 (MMP-9) in tumor tissues.Survival status was obtained by telephone and outpatient follow-up.The prognostic value of molecular markers was evaluated in patients with high-grade glioma patients.Results:Ki-67 and MGMT expression had significant effect on PFS (P<0.05),while MMP-9,EGFR and p53 had no effect on PFS (P>0.05).Multivariate Cox proportional hazard model analysis showed that Ki-67 was an independent prognostic factor affecting PFS (RR=5.19,P<0.05).Conclusion:Ki-67 is an independent prognostic factor of PFS in patients with high-grade glioma,which may have potential application value in prognostic evaluation.
