湖南省首例新型冠状病毒肺炎死亡病例流行病学调查分析

目的 了解湖南省首例COVID-19死亡病例的感染来源和死亡原因。 方法 按照《新型冠状病毒感染的肺炎防控方案》(第三版)要求,通过现场流行病学调查,追踪传染源,采集病例和可能传染源的鼻咽拭子,应用Real-time RT-PCR检测新型冠状病毒核酸,追踪排查密切接触者并集中隔离进行医学观察,对病例的流行病学和临床诊疗资料进行分析。 结果 该病例2020年1月21日与有“武汉旅行史”的COVID-19病例有短暂接触史,1月26日出现畏寒、发热,2月4日确诊新型冠状病毒肺炎,既往有贫血、高血压、肾功能不全、右肾萎缩史,发病前19 d有输尿管植入术史,发病至死亡全程均有腹泻史,2月8日死亡,为湖南省首例COVID-19死亡病例。 结论 湖南省首例COVID-19死亡病例的感染来源与有“武汉旅行史”的COVID-19病例高度关联,患有基础性疾病是导致该病例死亡的重要原因。

Epidemiological survey on the first dead case of COVID-19 in Hunan Province

Objective To analyze the source of infection and cause of death in the first dead case of COVID-19 in Hunan Province. Methods According to the requirements of National Novel Coronavirus Pneumonia Prevention and Control Scheme (the 3rd Edition), field epidemiological methods were used to trace the source of infection and collect the nasopharyngeal swabs of the patient and the suspected infection source to detect the 2019-nCoV viral nucleic acid by real-time RT-PCR. All of the close contacts were isolated at the assembly sites for medical observation. The epidemiological and clinical diagnosis and treatment data of the case were analyzed. Results On January 21, 2020, the patient had a transient contact with a COVID-19 case having a history of travelling in Wuhan, and developed chill and fever on January 26. The patient was diagnosed as a confirmed COVID-19 case on February 4. The patient suffered from anemia, hypertension and renal insufficiency, and had a history of right kidney atrophy and a surgical history of ureter implantation 19 days before onset. The patient had a history of diarrhea during the whole course of the disease, and died on February 8, regarding as the first dead case of COVID-19 in Hunan Province. Conclusion The first dead case of COVID-19 in Hunan Province had a high correlation with the COVID-19 patient with a history of travelling in Wuhan. Suffering from underlying diseases was the important cause of death in the first dead case.
