腹腔镜胸骨前径路甲状腺切除术的应用体会

目的总结腹腔镜胸骨前径路甲状腺切除术的临床应用经验。方法对12例甲状腺肿瘤患者行腹腔镜下胸骨前径路手术切除。结果除1例甲状腺腺瘤因瘤体与舌下肌群粘连严重而中转传统手术外，其余11例手术顺利，行一侧腺叶切除10例，另1例附加峡部切除。平均手术时间150（120～240）min，平均出血量60（50～80）ml，术后无声嘶、呛咳、大出血等严重并发症。1例术后出现胸骨切迹处皮下积液，经抽液和加压包扎后治愈。结论腹腔镜下胸骨前径路甲状腺切除术安全可行，疗效可靠，颈部美容效果好。

Laparoscopic Thyroidectomy via Anterosternal Approach

Objective To summarize the experience of laparoseopic thyroidectomy via anterosternal approach. Methods 12 cases of thyroid tumor underwent laparoseopic thyroidectomy via anterosternal approach. Results Apart from l converse case due to severe adhesion between tumor and hypoglossal muscle, the rest 11 cases were done successfully including unilateral thyroideetomy in l0 cases and additional isthmusectomy in 1 case. Average operative duration was 150 min, average blood loss was 60 ml. No hoarseness, bucking, massive hemorrhage occurred postoperatively, and 1 case had subcutaneously hydrops near sternal notch and cured by needle aspiration and pressure dressing. Conclusion Laparoscopie thyroideetomy via anterosternal approach is a safe, feasible and effective procedure with good cosmetic result.
