男性青少年肥胖伴乳房发育症性激素和受体测定及三苯氧胺疗效观察

目的:探讨性激素及雌、孕激素受体(ER,PR)在男性青少年肥胖伴乳房发育症(OGMA)发病中的作用及三苯氧胺的疗效。方法:应用放射免疫分析法测定20例OGMA患者的血清性激素水平,免疫组化法检测其中10例乳腺组织ER、PR含量,并行三苯氧胺治疗。结果:OGMA患者血清总睾酮(TT)、睾酮/雌二醇(T/E2)降低,ER、PR阳性率均为80%。13例患者三苯氧胺治疗3个月,乳腺无明显缩小。结论:肥胖相关的血清TT、T/E2下降与OGMA发生密切相关,ER、PR含量增加是发病机制之一,三苯氧胺治疗本症疗效不确切。

Sex hormones and receptors study of obesity with gynecomastia in male adolescents and treatment with tamoxifen

Objective:To study the roles of sex hormones and receptors play in obesity with gy-necomastia in male adolescents(OGMA),and the therapy effect of tamoxifen.Methods:Serum LH,FSH,to-tal testosterone(TT),estradiol (E 2 ),PRL concentrations of 20cases were measured by RIA,and estrogen re-ceptor (E R)and progesterone receptor (P R)contents of breast tissues taken from10cases were determined by immunohistochemical method.13cases were given tamoxifen.Results:Levels of serum TT and T /E 2 were less than those in the control groups,and both ER and PR positive rates were80%.After treatment for three months,the sizes of gynecomastia showed no significant decrease.Conclusion:Sex hormone imbal-ance related to obesity and the overexpression of ER and PR play roles in the pathogenesis of OGMA.We can't draw a conclusion about the therapy effect of tamoxifen now.
