老年骨质疏松性股骨粗隆间骨折的手术治疗经验

目的 探讨老年骨质疏松性股骨粗隆间骨折的手术经验。方法 对79例老年骨质疏松性股骨粗隆间骨折患者行内固定治疗(63例)和骨水泥型双极人工股骨头置换术(16例)。结果 行内固定治疗者患肢功能的优良率为80．9％(51／63例)，并发症发生率多。行双极人工股骨头置换术者患肢功能的优良率为93．8％(15／16例)。结论 对无手术禁忌证的老年骨质疏松性股骨粗隆间骨折应尽可能行假体置换术，可以提高患者的生活质量，减少并发症。

The experience in the operation of intertrochanteric hip fracture in the elderly

Objective To appraise the experience in the operative treatment for intertrochanteric hip fracture in the elderly. Methods Among the 79 elder patients with intertrochanteric fractures, 63 were treated with internal fixation and 16 with bipolar prosthetic replacement. Results The hip function of internal fixation treatment was excellent to good in 80.9%(51/63), but with higher complication rate. The hip function of bipolar prosthetic replacement treatment was excellent to good in 93.8%(15/16). Conclusion For elder patients with osteoporotic intertrochanteric hip fracture without operation contraindication, bipolar prosthetic replacement should be the priority choice to improve the quality of life and decrease the complication rate.
