胃癌合并穿孔20例临床分析

目的　探讨胃癌合并穿孔的诊断和手术处理原则。方法　回顾性统计分析我院近 10年胃癌合并穿孔 2 0例的诊断、手术治疗的时机和术式选择。结果　单纯穿孔缝合修补术 8例 ,平均生存时间为 3个月 ;姑息性胃大部切除术 6例 ,平均生存时间为 1年 ;根治性胃大部切除和全胃切除术 6例 ,平均生存时间 4年余。结论　胃癌合并穿孔的手术方式有多种 ,如能掌握合适的指征和方法 ,根治性的切除能明显提高生存率。

Clinical analysis of 20 cases of perforating gastric cancer

Objective To investigate the diagnosis and surgical treatment principles of perforating gastric cancer.Methods The diagnosis, opportunity and modes of surgical treatment in 20 cases of perforating gastric cancer were retrospectively statistically analyzed in recent ten years in our hospital.Results Eight cases received simple suture with the average survival time being three months, 6 palliative subtotal gastrectomy with the average survival time being one year, and 6 radical subtotal gastrectomy or total gastrectomy with the average survival time being 4 years. Conclusion There are many surgical precedures for perforating gastric cancer. In the light of appropriate indication and methods, radical resection could obviously increase the survival rate.
