延伸护理对居家腹膜透析患者腹膜炎发生率及病情预后的影响

目的探讨延伸护理应用于居家腹膜透析患者的临床疗效,并为该类患者提高新型护理服务。方法选取我院于2011年6月至2013年12月收治的96例行居家腹膜透析治疗的肾功能衰竭患者,采用随机数字表法随机分为研究组和对照组,每组各48例,其中对照组采取常规护理方法及措施,研究组则在此基础上采取延伸护理服务,主要包括随访及家庭护理。比较两组患者的腹膜炎发生率、住院率及不良事件发生率。结果研究组腹膜炎发生率及再次住院率均低于对照组（P〈0.05）。研究组水负荷致死、血钾异常及收缩压〉140 mm Hg发生率低于对照组,差异有统计学意义（P〈0.05）。结论延伸护理能有效降低居家腹膜透析患者腹膜炎发生率,提高患者治疗依从性,延长了患者生存时间,具有良好的治疗效益。

Effects of extended care on the incidence and prognosis of peritonitis in patients undergoing peritoneal dialysis at home

Objective To investigate the clinical efficacy of extended care for patients with peritoneal dialysis at home, and to improve the model of nursing service for the patients. Methods Ninety-six patients of renal failure undergoing peritoneal dialysis at home admitted in our hospital from Jun. 2011 to Dec. 2013 were randomly divided into the study group and the control group, with 48 cases in each group. The control group received routine nursing care methods and measures, while the study group received extended care（including follow-up and family nursing） on the basis of the control group. The incidence of peritonitis, hospitalization rate and the incidence of adverse events were compared between the two groups. Results The incidence of peritonitis and hospitalization rate in the study group were significantly lower than those in the control group（P0.05）. The water load lethal, potassium abnormality and the incidence of systolic blood pressure 140 mm Hg the study group were significantly lower（P0.05）. Conclusion Extended care can effectively reduce the incidence of peritonitis in patients undergoing peritoneal dialysis at home, and improve the patients compliance, prolong the survival time of patients, which has good clincial effect.
