| 摘 要： | 目的评价椎弓根螺钉内固定治疗不稳定性Hangman骨折的临床应用价值。方法回顾分析我科自2002年以来收治的9例不稳定性Hangman骨折患者，其中Ⅱ型5例，Ⅱa型2例，Ⅲ型2例，所有患者均采用C2～3椎弓根螺钉侧块钉板系统内固定治疗。结果经过12～38个月随访，X线片显示骨折愈合，复位良好。无动脉损伤及其他手术并发症。1例术前Frankel分级为D级的患者术后恢复至E级。结论椎弓根螺钉内固定治疗不稳定性Hangman骨折可获得骨折即时复位，固定可靠，是一种理想的治疗方法。

| Abstract: | Objective To evaluate the outcomes of pcdicle screw fixation in treatment of unstable Hangman＇s fracture. Methods 9 cases with unstable Hangman＇s frature were retrospectively analyzed. 5 of them were type Ⅱ ,2 of them were type Ⅱ a,and 2 of them were type Ⅲ. All patients were treated with pedicle screw fixation. Results After 12 to 38 months following-up,X-ray showed all the fractures healed up with satisfactory reduction.No vertebra artery injury or other complications occurred. 1 case of grade D according to frankle classification recovered to grade E after operation. Conclusion Pedicle screw fixation is an ideal way of treatment of unstable Hangman＇s fracture,which can provide immediate reduction and satisfactory biomechanical stability. |
