小儿重症肺炎血小板参数的动态变化分析

摘    要:目的:探讨重症肺炎患儿血小板参数动态变化。方法:动态测定38例重症肺炎患儿(A组)的血小板参数包括血小板计数(PLT)、血小板分布宽度(PDW)、平均血小板容积(MPV)及大血小板比率(P-LCR),同时与普通肺炎患儿80例(B组)和正常儿童(C组)100例作对照。结果:入院24h内A组PLT明显高于B、C组,均P<0.01,而PDM、MPV、P-LCR低于C组,差异有显著性,P<0.05;B组PLT高于C组,P<0.05,而PDW、MPV、P-LCR与C组比较无差异,P>0.05。入院72h后复查,B组各血小板参数基本恢复正常,与C组比较无差异,P>0.05;A组PLT明显下降,而PDM、MPV、P-LCR明显升高,与C组比较有显著性差异,P<0.05;入院第7d复查,各参数基本恢复正常。结论:动态检测肺炎患儿的血小板参数有助于准确评估病情的危重度。

Abstract:Objective: To investigate the dynamic changes of platelet parameters in children with severe pneumonia. Methods: Dynamic changes of platelet count(PLT),platelet distribution width(PDW),mean platelet volume(MPV)and platelet-large cell ratio(P-LCR) were detected in 38 children with severe pneumonia(group A), 80 with general pneumonia(group B) and 100 healthy controls(group C). Results: PLT in group A was significantly higher than group B and group C,P < 0.01, while PDW?MPV?P-LCR were significantly lower than group C in 24 hours,P < 0.05. PLT in group B was significantly higher than group C,P < 0.05, but PDW?MPV?P-LCR were no differences between group B and C,P > 0.05. After 72 hours, the parameters of platelet in group B were recovered. PLT in group A was significantly decreased but PDW?MPV?P-LCR were significantly increased on day 3 and all the parameters of platelet in group A become normalized on day 7. Conclusions: Simultaneous detection of platelet parameters is helpful in evaluating the severity of pneumonia.
