“部分蛋壳”技术在胸腰段脊柱后凸畸形后路椎间隙楔形截骨矫形术中的初步应用

目的探讨在后路椎间隙楔形截骨矫形术中应用后路经椎弓根“部分蛋壳”技术治疗胸腰段脊柱后凸畸形的早期临床效果。方法2004年9月～2006年10月，在后路椎间隙楔形截骨矫形术中应用“部分蛋壳”技术治疗不同病因所致胸腰段脊柱后凸畸形9例。术前脊柱后凸Cobb角平均为71°（45°～93°）。采用后路经椎弓根“部分蛋壳”技术，对顶椎和／或相邻楔形变椎体予以楔形截骨。结果术后1例出现一过性单侧神经根功能障碍，1例肺炎。脊柱后凸Cobb角平均为12．4°，平均矫正度数为58．6°，平均矫正率为82．5％。结论后路经椎弓根“部分蛋壳”技术能够在充分保护脊髓和神经根的前提下实现对椎间盘和部分椎体的切除，在胸腰段脊柱后凸畸形后路椎间隙楔形截骨矫形术中有一定应用价值。

Transpedicular part eggshell technique to the posterior intervertebral angulation osteotomy correct the thoracolumbar kyphosis

Objective To investigate the early result of a transpedicular part eggshell technique to the posterior intervertebral angulation osteotomy in thoracolumbar kyphosis. Methods From 2004 to 2006, 9 cases were treated by a transpedicular part eggshell technique in the posterior intervertebral angulation osteotomy for thoracolumbar kyphosis. The average preoperative Cobb's angle of kyphosis was 71°(rang from 45°to 93°). All the cases were treated by a transpedicular part eggshell technique during the posterior intervertebral angulation osteotomy. Results One case had temporary dysfunction of one lower extremity, 1 case had pneumonia. The average postoperative Cobb's angle of kyphosis is 12.4°. The average deformity correction angle was 58.6°with the average deformity correction rate up to 82.5%. Conclusion Transpedicular part eggshell technique can resect intervertebral disc and part body, with the protection of spinal cord and nerve roots,so it is valuable in the posterior intervertebral angulation osteotomy for thoracolumbar kyphosis.
