探讨多普勒超声、钼靶X线及MRI在乳腺癌诊断中的应用价值

摘    要:目的:研究并探讨多普勒超声、钼靶X线、乳腺MRI在乳腺癌诊断中的应用价值。方法整群选取2015年1—8月收治的69例疑似乳腺癌患者进行研究。对这69例患者分别进行多普勒超声检查、钼靶X线检查以及乳腺MRI检查,并以术后病理诊断结果作为标准,计算超声+钼靶+MRI、超声+MRI的敏感度、特异度、准确性、阳性预测值、阴性预测值。结果69例疑似乳腺癌患者中,经术后病理诊断,57例患者被确诊为乳腺癌。影像学检查结果显示,超声+MRI的敏感度为80.70%,特异度为58.33%,准确性为76.81%,阳性预测值为90.20%,阴性预测值为38.89%;超声+钼靶+MRI的敏感度为92.98%,特异度为83.33%,准确性为91.30%,阳性预测值为96.36%,阴性预测值为71.43%,其中两种联合检查方法的敏感度、特异度、阳性预测值、阴性预测值,差异无统计学意义(χ2值分别为3.762、1.815、1.632、3.348,P>0.05),而两组的准确性、阴性预测值,差异有统计学意义(χ2值为5.408,P<0.05)。结论在临床诊断时,应对患者进行乳腺MRI、多普勒超声、钼靶X线联合检测,以提高乳腺癌的检出率和诊断准确性,减少漏诊和误诊的发生。

Study on Application Value of Doppler Ultrasound,Molybdenum x-ray and MRI in the Diagnosis of Breast Cancer
Abstract:Objective To research and study the application value of Doppler ultrasound, molybdenum x-ray and MRI in the diagnosis of breast cancer. Methods 69 cases of patients with suspected breast cancer from January 2015 to August 2015 were selected and given Doppler ultrasound examination, molybdenum x-ray examination and breast MRI examination, and the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of ultrasound plus molybde-num plus MRI and ultrasound plus MRI were calculated according to the postoperative pathological diagnosis results. Re-sults After postoperative pathological diagnosis, 57 cases were diagnosed as breast cancer of the 69 cases with suspected breast cancer, Imaging examination results showed that the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were respectively 80.70%, 58.33%, 76.81%, 90.20% and 38.89% of ultrasound plus MRI and 92.98%, 83.33%, 91.30%, 96.36% and 71.43% of ultrasound plus molybdenum plus MRI, there was no obvious difference in the sensitivity, specificity, positive predictive value and negative predictive value between the two combined examination methods (the Chi-square values:3.762, 1.815, 1.632, 3.348, P>0.05), the differences were not statistically significant, there was an obvious difference in the accuracy between the two groups (the Chi-square value:5.408, P<0.05), the difference was statistically significant. Conclusion We should use the combined examination of breast MRI, Doppler ultrasound and molybdenum x-ray in clinical diagnosis in order to improve the detection rate and diagnostic accuracy of breast cancer and decrease the occurrence of misdiagnosis and missed diagnosis.
