家庭医生签约服务利用与满意度的比较研究

目的:比较家庭医生签约服务中签约居民与非签约居民对于社区卫生服务利用与满意度的差异,依此验证签约家庭医生对于社区卫生服务利用与满意度的影响。方法:采用问卷调查采集相关数据建立数据库。主要使用卡方检验、因子分析、多元线性回归分析等统计方法。结果:签约居民对于社区卫生服务的利用显著高于非签约居民,满意度的各项指标也普遍高于非签约居民。在控制了基本人口学变量后,满意度综合指标的回归结果也表明是否签约是影响满意度的核心解释变量。结论:家庭医生制度的实施提高了社区卫生服务的利用,进而促成分级诊疗的有序医疗格局。服务利用提升的同时也将有助于提高居民对于基本医疗卫生服务的获得感与满足感。

A comparative study on the health services utilization and satisfaction of the family doctor service

Objective: To test the effect of the Family Doctor on Community Health Services utilization and satisfaction, by comparing the difference between those who have signed the service contract and those who have not. Methods: To collect data and create the database, a questionnaire survey was conducted. To deal with data using Chi-square test, Factor Analysis, Multiple Linear Regression and other statistical methods were adopted. Results: Community health services utilization is significantly higher for the contracted residents than in non-signatory ones, a case which is the same with the satisfaction. Besides, after controlling for the basic demographic variables as gender, age, education et al., the regression composite indicators with overall satisfaction and its impacts also shows the same result. Conclusion: The implementation of the Family Doctor System has increased the utilization of Community Health Services, and thus contributes to the orderly hierarchical medical system. Therefore, a massive utilization of health services could help to improve the benefit sense and satisfaction of residents.
