目的改善慢性胰腺炎的治疗效果？方法回顾性分析1985～2000年收治的58例慢性胰腺炎病人的治疗效果。结果33例接受非手术治疗的病人多数有效，但多有复发。经手术治疗的25例病人效果明显好于非手术治疗组。结论对胰头包块型慢性胰腺炎、梗阻型慢性胰腺炎、假性胰腺囊肿应手术治疗；对病变弥漫且无胰管梗阻者，应采取内科治疗，若药物难以控制，可采用胰腺去神经术；对合并胆石的慢性胰腺炎，应积极进行手术处理。

Effects of different managements for chronic pancreatitis

Objective To improve the clinical outcome of treatment for chronic pancreatitis.Methods Fifty-eight patients with chronic pancreatitis receiving treatment in our hospital from 1985 to 2000 were analyzed retrospectively.Results Thirty-three patients received nonoperative treatment,and 25 patients were managed by surgical treatment.Both kinds of treatment were effective with easier relapsing in group managed by nonoperative treatment and a significantly better effect of surgical treatment.Conclusion The clinicopathologic characteristics of chronic pancreatitis is variable and the management should also be different for individuals.Resection should be performed for patients with inflammatory mass in the head of pancreas.When a dilated ductal system is encountered,drainage procedures should be recommended.Cystojejunostomy should be performed for pancreatic pseudocyst.Internal medicine treatment was suitable for chronic diffuse inflammation of pancreas without dilated duct.Denervation should be recommended when internal medicine treatment becomes ineffective.Cholelithiasis with chronic pancreatitis should be managed by surgical treatment earlier.
