慢性阻塞性肺疾病患者外周血单个核细胞核因子-κB水平及与气流受限的关系

摘    要:目的:探讨单个核细胞核因子-κB(NF-κB)在慢性阻塞性肺疾病(COPD)发病机制中的作用及其与COPD急性加重期(AECOPD)患者气流受限的关系。方法:选择AECOPD患者共60例,根据其气流受限程度分为:轻-中度AECOPD组(30例)、重-极重度AECOPD组(30例)。采用蛋白免疫印迹法(Western blotting法)和酶联免疫吸附测定(ELISA)法分别检测观察组及健康对照组(30例)外周血单个核细胞核内NF-κB水平及IL-8、IL-10含量。结果:AECOPD组外周血单个核细胞核内NF-κB水平及IL-8含量高于对照组(p<0.01、p<0.05),IL-10含量低于对照组(p<0.01);重-极重度AECOPD组外周血单个核细胞核内NF-κB水平及IL-8含量高于轻-中度组(p<0.01、p<0.05),IL-10含量低于轻-中度组(p<0.01)。外周血单个核细胞核内NF-κB水平与血清IL-8含量呈正相关(r=0.84,p<0.05),与血清IL-10含量及FEV1占预计值%呈负相关(r=-0.78,r=-0.97,p均<0.05)。结论:NF-κB参与COPD的气道炎症,与COPD患者气流受限呈正相关。

Abstract:Objective:To explore the effect of nuclear factor-κB(NF-κB) in peripheral blood mononuclear cells nucleus on the pathogenesis of chronic obstructive pulmonary disease(COPD) and the relationship with the airflow limitation in the patients with acute exacerbation of COPD(AECOPD).Methods: 60 AECOPD patients were divided into two subgroups according to their airflow limitation degree: light-moderate AECOPD group(30 case),severe-very severely AECOPD group(30 cases).Western blotting method and enzyme-linked immunosorbent assay(ELISA) were adopted to respectively detect the levels of NF-κB in peripheral blood mononuclear cells nucleus and IL-8,IL-10 in serum.Results: The levels of NF-κB in peripheral blood mononuclear cells nucleus and IL-8 in serum of AECOPD group were both higher than that in the control group(p<0.01,p<0.05),the levels of IL-10 in serum were lower than that in the control group(p<0.01);The levels of NF-κB and IL-8 in the severe-very severely group were both higher than those in the light-moderate group(p<0.01,p<0.05),the level of IL-10 was lower than that in the light-moderate group(p<0.01).The level of NF-κB showed a positive correlation with the level of IL-8(r=0.84,p<0.05),and showed a positive correlation with the level of IL-10 and FEV1% predicted data(r=-0.78,p<0.05,r=-0.97,p<0.05).Conclusions: NF-κB participated in airway inflammatory response of COPD shows a positive correlation with the degree of airflow limitation.
