颅内动脉瘤64层螺旋CT血管成像的质量控制探讨

摘    要:目的探讨颅内动脉瘤64层螺旋CT血管成像的图像后处理技术及其质量控制。方法采用东芝Aquilion64层螺旋CT机对21例怀疑颅内动脉瘤的患者行容积扫描,在VITREA2工作站对数据分别行容积再现技术(VR)、最大密度投影(MIP)、二维多平面重建(MPR)、曲面重建(CPR)、仿真内窥镜(VE)等图像后处理,后由3名中级以上职称医生结合原始横断面图像进行对比分析。结果21例患者均扫描成功,原始图像效果满意。21例患者中MSCTA发现动脉瘤19个,其中大脑中动脉6个,大脑前动脉7个,大脑后动脉4个,颈内动脉2个。2例海绵窦旁动脉瘤漏诊。MSCTA上动脉瘤显示直径3~28mm不等。结论多种图像后处理技术对颈动脉不同病变的显示各有优势,综合使用可以提高颅内动脉瘤的检出率。

Abstract:Objective To investigate the post processing methods and quality control in cerebral aneurysms with 64-slice spiral CT angiography. Methods 21 patients with suspected cerebral aneurysms were carried out volume scan by Toshiba Aquilion 64-slice spiral CT. All the raw data were transmitted to the VITREA2 work station and were reconstructed using VR,MIP MPR,CPR and VE. All the original images at transect were viewed and compared by 3 doctors. Results 19 cerebral aneurysms were found by MSCTA,6 aneurysms were located in the middle cerebral artery,7 in the anterior cerebral artery,4 in the posterior cerebral artery and 2 in the internal carotid artery. 2 paracavernous sinus aneurysm were missed on MSCTA. The diameter showed different data between 3~20mm on MSCTA images. Conclusion Different post processing methods have different advantages in displaying the cerebral aneurysms. Original images,MPR,MIP combined with 3D CTA can improve the detectable rate of cerebral aneurysms.
