小儿食管异物86例临床分析

目的探讨对小儿食管异物处理的临床经验。方法对收治的小儿食管异物86例，在表麻或无麻下进行食管镜探查并异物取出术的临床资料分析。结果手术中成功取出异物66例(76．740,6)，将异物推入胃内12例(14．28％)，术中未发现异物8例(0．93％)，但术后经X线检查证实这8例脱落于胃内，20例落入胃内的异物，经口服石蜡油后，在24～72h内全部排出体外。4例并发食管穿孔，经禁食及鼻饲管，积极抗炎及支持疗法而痊愈。结论小儿的食管异物要及时诊断，及早取出，手术操作要轻巧，尽量减少对食管黏膜的损伤，对难取出的异物可推入胃内让其随大便排出，避免并发症的发生。

86 clinical analysis of child'' s foreign bodies in esophagus

Objective To probe into the clinical experience of treating child's foreign bodies in esophagus.Methods 86 cases of child's foreign bodies in esophagus that accepted for medical treatment were carried on ascertaining foreign bodies in esophagus by esophagus mirror to take out and made the clinical analysis of the operation under the circumstances that the mucous membrane of esophagus is anaesthetized or not anaesthetized.Results 66 cases of foreign bodies in esophagus were succeed in taking out in the operation(account for 76.74%),12 cases foreign bodies in esophagus were pushed into the stomach(account for 14.28%),8 cases had not found foreign bodies in the operation(account for 0.93%).They were verified to check by X-ray that these foreign bodies were coming off into the stomach,20 cases foreign bodies that falling into the stomach were discharged outside the body after oral paraffin oil in 24 to 72 hours,4 cases were found to complicate esophagus boring a hole,after fasting and nasal feeding,resisting the inflammation and supporting the treatment and they were fully recovered.Conclusion The child's foreign bodies in esophagus must be diagnosed and taken out early in time,and operated lightly and handily at the time of the operation,and try one's best to reduce the damage to the mucous membrane of esophagus,to foreign bodies in esophagus which were difficult to take out,it can push and let it be discharged through stool in the stomach.It can avoid the emergence of complications.
