间苯三酚在冻融胚胎移植中的应用

摘    要:目的:探讨体外受精-胚胎移植(IVF-ET)术前应用间苯三酚增加临床妊娠率效果。方法:前瞻性随机对照,将2015年3~7月在本生殖中心接受IVF-ET≤35岁282人随机分为2组,间苯三酚组IVF-ET前3min静脉推注间苯三酚80mg,对照组术前不使用间苯三酚。助孕指征包括女方、男方或男女双方复合因素,不明原因不孕等。剔除子宫内膜7mm或反复种植失败,比较两组的临床妊娠率、流产率和异位妊娠率。结果:两组年龄、BMI、不孕年限、移植胚胎数、移植优胚数、子宫内膜厚度等均无差异。按移植优胚数分层,移植1个优胚时间苯三酚组的种植率高于对照组(33.6%比14.5%,P0.01),临床妊娠率高于对照组(49.1%比26.9%,P0.05)。移植2个优胚时间苯三酚组和对照组的种植率、临床妊娠率(46.4%比38.7%,68.9%比59.4%)差异均无统计学意义,两组异位妊娠率和流产率均无差异(P0.05)。结论:移植1个胚胎情况下,IVF-ET前静脉推注间苯三酚有助于提高胚胎种植率和临床妊娠率。

Abstract:Objective:To explore phloroglucinol increasing the rate of clinical pregnancy before in vitro fertilization and embryo transfer(IVF-ET) using frozen-thawed embryo.Methods:A prospective randomized controlled study was conducted from March 2015 to July 2015,282 patients who were equal to or under 35 years old and had undergone IVF-ET were allocated to the study group and the control group.The patients in the study group had received an intravenous injection of 80mg phloroglucinol 3min before embryo transfer,but patients in the control group needn't given phloroglucinol.Indicators of IVF-ET included factors of wives only,husbands only,neither wives and husbands,or unexplained infertility.The excluded criteria included peak endometria-thickness less than 7.0mm and repeatedly planting failure.Clinical pregnancy rate,implantation rate,miscarriage rate and ectopic pregnancy rate were compared between the two groups.Results:There were no statistical difference between of age,BMI,years of infertility,number of embryos transferred,number of high-quality embryos transferred,and endometrial thickness between the two groups.When 1 high-quality embryo was transferred per IVF-ET cycle,the implantation rate and clinical pregnancy rate of patients in the study group were significant higher than those of patients in control group (33.6 Vs.14.5%,49.1%vs.26.9 %,P<0.05).When 2 high-quality embryos were transferred per IVF-ET cycle,although the implantation rate and clinical pregnancy rate of patients in the study group were higher than those of patients in control group (46.4 % vs.38.7 %,68.9 % vs.59.4 %),there were no significant difference(P >0.05).Conclusion:Intravenous injection of phloroglucinol before IVF-ET is beneficial to increase implantation rate and clinical pregnancy rate when1 high-quality embryo is transferred.
