非肥胖型PCOS不孕患者超声检测肥胖指标的临床价值

目的:探讨非肥胖型多囊卵巢综合征(PCOS)患者超声检查肥胖相关指标的临床价值。方法:选取2016年1月-2017年1月本院就诊的不孕患者1180例,其中非肥胖型PCOS患者590例(PCOS组),非PCOS患者590例(非PCOS组)。测量两组患者的基础性激素、腹部皮下脂肪、肝前脂肪、肝前皮下脂肪、内脏脂肪,以及身高、体重、腰围、腹围等,并对比分析。结果:PCOS组腹围大于非PCOS组(P0.05),腰围、臀围及腰臀比两组比较无差异(P0.05);内脏脂肪厚度和皮下脂肪厚度(脐上1cm处)PCOS组大于非PCOS组(P0.05),肝前脂肪厚度和肝前皮下脂肪厚度两组间无差异(P0.05);PCOS组黄体生成素、睾酮、、空腹血糖、空腹胰岛素值及胰岛素抵抗指数值均大于非PCOS组(P0.05),而卵泡刺激素、泌乳素和雌二醇水平两组间无差异(P0.05)。结论:非肥胖型PCOS患者虽然体重指标在正常范围内,但是腹部皮下脂肪、肝前脂肪、肝前皮下脂肪、内脏脂肪等超声测量指标大于非PCOS患者,在临床治疗不孕症时应引起重视。

Valuation analysis of obesity related indexes measured by ultrasonography for infertile women with non-obese PCOS

Objective:To explore the valuation of obesity related indexes measured by ultrasonography for infertile women with non-obese PCOS.Methods:1180 women treated in Guangzhou H uadu District Maternal and Child Health Care Hospital from January 2016 to January 2017 were included and divided into NOB-PCOS group (590cases) and NON-PCOS group (590cases).The indexes,such as basis sex hormones,abdominal subcutaneous fat,liver fat,subcutaneous fat before liver,visceral fat,as well as the height,weight,waist circumference,hip circumference and abdomen circumference of women had been compared between the two groups.Results:The abdominal circumference of women in NOB-PCOS group was statistically significantly higher than that of women in NON-PCOS group(P＞0.05),but there were no statistical significant difference in abdominal subcutaneous,hip circumference and waist-hip ratio between the two groups(P＜0.05).Subcutaneous fat (1 cm above the umbilicus) and visceral fat thickness of women in NOB-PCOS group were significant higher than those of women in NON-PCOS group (P＜0.05).There were no statistical significant difference in prehepatic fat thickness and prehepatic subcutaneous fat thickness between the two groups (P ＞0.05).The level of Luteinizing hormone(LH),testosterone(T),fasting blooding glucose(FBG),fasting insulin (FINS),and HOMA-IR in serum of women in NOB-PCOS group were statistical significant higher than those of women in NON-PCOS group (P＜0.05),but There were no statistical significant difference in level of follicle-stimulating hormone(FSH),prolactin and estradiol of women between the two groups.Conclusion:Although the body mass index (BMI) of women with NOB-PCOS are normal,the abdominal circumference abdominal subcutaneous fat thickness and visceral fat thickness are higher than those of infertile women without PCOS,which should be paid more attention to when clinical treating infertility.
