腹腔镜胃十二指肠溃疡穿孔修补术的临床研究

目的探讨腹腔镜在胃十二指肠溃疡穿孔修补术中的作用。方法将76例胃十二指肠溃疡穿孔患者随机分为腹腔镜组和传统组,每组各38例。传统组患者采用传统开腹修补术,腹腔镜组采用胃十二指肠溃疡腹腔镜穿孔修补术,比较两组患者的手术效果和术后并发症发生情况。结果腹腔镜组患者在手术时间、术中出血量、肛门排气时间、下床活动时间、住院时间及并发症发生率等指标上均比传统组低(P0.05),且在术后第3天感到中重度疼痛的患者比率低于传统组(P0.05)。结论胃十二指肠溃疡腹腔镜穿孔修补术与传统开腹手术比较,手术效果更好,患者舒适感高,值得推广。

Clinical study on laparoscopic gastroduodenal ulcer perforation repair

Objective To study the value of laparoscopic repair for perforated gastroduodenal ulcer.Methods 76 cases of gastroduodenal ulcer complicating perforation were randomly divided into laparoscopic and open repair group,38 cases for each. The therapeutic effect and postoperative complications were compared between two groups. Results The operation time,intraoperative blood loss,postoperative anal exsufflation time,the time of out-of-bed activity,hospital stay and complication rate were lower in the laparoscopic group than in the open group（ P 〈 0. 05）,so were the proportion of moderate and severe pain on the third day after peration（ P 〈 0. 05）. Conclusion Compared with open surgery,laparoscopic repair for perforated gastroduodenal ulcer has the advanges of better effect and less postoperative pain,worthy of clinical promotion.
