非霍奇金淋巴瘤化疗疗效影响因素分析

目的:分析影响非霍奇金淋巴瘤化疗疗效的主要因素。方法:选择初治非霍奇金淋巴瘤(NHL)患者135例,病理诊断为T或B细胞NHL,分别按T或B细胞病理类型、国际预后指数(IPI)≤2或>2、有无结外侵犯、肿瘤直径≤5cm或>5cm各分两组,所有患者给予CHOP方案化疗3次,分别比较各组的总有效率。结果:B细胞型NHL总有效率(75%)和T细胞型NHL总有效率(55.8%),IPI≤2组NHL总有效率(73.4%)和IPI>2组NHL总有效率(52.1%),无结外侵犯组NHL总有效率(73.8%)和有结外侵犯组NHL总有效率(42.9%)的差异有统计学意义。肿瘤直径≤5cm组总有效率(69.9%)与肿瘤直径>5cm组NHL总有效率(50%)差异无统计学意义。结论:肿瘤病理类型、IPI和结外器官侵犯可能响NHL化疗疗效的因素,肿瘤直径可能不是影响NHL化疗疗效的因素。

Analysis of factors influencing the efficacies of chemotherapy for non-Hodgkin's lymphoma

Objective：To analyze the main factors influencing the efficacies of CHOP regiment in treating non-Hodgnin’s Lymphoma （NHL）.Methods：135 patients first diagnosed as T-cell NHL or B-cell NHL were observed.They were divided respectively into two groups according to pathologic type,international prognostic index （IPI）,extranodal involvement,tumor diameter.All patients received CHOP regiment at 3 cycles.The total response rates （RR） of each group containing complete remission （CR） rate and partial remission （PR） rate were compared respectively.Results：The differences of total response rates （RRs） between B-cell NHL group （75%） and T-cell NHL group （55.8%）,the group of IPI less than or equal to 2 （73.4%） and the group of IPI more than 2 （52.1%）,no extranodal involvement group （73.8%）and extranodal involvement group（42.9%）were significant.The RR at group of tumor diameter less than 5 cm was 69.9%,the RR at group of tumor diameter more than 5 cm was 50%,the difference between two groups was no significant.Conclusion：These data shows that pathologic type,IPI and extranodal involvement,except tumor diameter,are possible factors to influence the efficacies of chemotherapy for NHL.
