经支气管镜刷检和灌洗液检查对肺结核的诊断价值

目的探讨电子支气管镜检查对肺结核的诊断价值。方法对99例肺结核患者治疗前的电子支气管镜检查结果进行回顾性分析,计算刷检、灌洗液检查及刷检+灌洗液的三种不同取材方法检出的抗酸菌阳性率。结果刷检涂片、灌洗液涂片及刷检涂片+灌洗液涂片找出抗酸杆菌的阳性率分别为56.10%(46/82)、64.63%(53/82)、76.25%(61/80),刷检加灌洗组优于其他两组,差异有统计学意义(P0.05)。结论电子支气管镜检查对肺结核的确诊具有重要的临床应用价值,联合刷检和灌洗液两种取材方法可提高肺结核的诊断率。

The diagnostic value of bronchoscopic brush smear and lavage fluid checks in pulmonary tuberculosis

Objective To investigate the diagnostic value of electronic bronchoscopy in pulmonary tuberculosis. Methods Electronic bronchoscopic findings before treatment of 99 cases of pulmonary tuberculosis patients were retrospectively analyzed. The positive rates of diagnosis of pulmonary tuberculosis through different methods of bronchoscopic brush smear, lavage fluid checks, and the above combination were calculated. Results The positive rates of acid-fast bacilli was 56.10%, 64.63%,and 76.25% for bronchoscopic brush smear, lavage fluid checks, and the above combination, respectively, with a significant superiority of the combination over solitary brush and lavage. Conclusion Electronic bronchoscopy has important value in diagnosing pulmonary tuberculosis. The combination of bronchoscopic brush smear and lavage fluid checks can increase diagnostic rate of pulmonary tuberculosis significantly.
