微创经皮锁定加压钢板与动力髋螺钉治疗股骨转子间骨折的疗效比较

摘    要:目的比较微创经皮锁定加压钢板与动力髋螺钉治疗股骨转子间骨折的疗效。方法选择在该院进行治疗的股骨转子间骨折病人150例,随机分为微创经皮锁定加压钢板组(80例)和动力髋螺钉组(70例),对比两组患者的平均透视时间、手术时间、手术失血量、术后住院时间以及临床疗效。结果微创经皮锁定加压钢板组患者平均手术时间为(39±5)min,显著短于动力髋螺钉组的(73±7)min;微创经皮锁定加压钢板组的平均失血量为(120±19)mL,显著少于动力髋螺钉组的(320±23)mL;微创经皮锁定加压钢板组的术后住院时间为(2.5±1.0)周,显著短于动力髋螺钉组的(6.5±2.0)周,差异均具有统计学意义(P0.05)。但两组在平均透视时间及术后优良率这两方面差异无统计学意义(P0.05)。结论微创经皮锁定加压钢板技术治疗股骨转子间骨折可明显缩短手术时间,减少患者出血量,有效避免出血过多引发的一系列并发症,有利于患者早日康复。

Abstract:Objective To compare the curative effect of minimally invasive percutaneous locking compression rotor plate and dynamic hip screw for the treatment of femoral intertrochanteric fracture. Methods One hundred and fifty patients with femoral intertrochanteric fracture were randomly divided into minimally invasive percutaneous locking compression plate group (80 cases) or dynamic hip screw group (70 cases). Average fluoroscopy time, operation time, blood loss in operation, postoperative hospitalization time , and clinical efficacy were compared between the two groups. Results Average operation time was (39 ± 5) minutes in minimally invasive percutaneous locking compression plate group, significantly shorter than (73 ±7) minutes of the dynamic hip screw group; the mean blood loss was ( 120 ± 19) ml in minimally invasive percutaneous locking compression plate group, significantly less than (320 ± 23) ml of the dynamic hip screw group, and postoperative hospitalization time was (2. 5 ± 1 ) weeks in minimally invasive percutaneous locking compression plate group, significantly shorter than (6.5 ± 2) weeks of dynamic hip screw group (P 〈 0.05). No significant difference was observed in average fluoroscopy time or the fine rate between the two groups (P 〉 0.05). Conclusions Minimally invasive percutaneous locking can shorten the operation time, and reduce the amount of bleeding and its complications for femoral intertrochanteric fracture.
