关节镜联合经皮微创锁定钢板固定治疗胫骨平台骨折的疗效分析

目的探讨关节镜联合经皮微创锁定钢板固定(minimally invasive percutanenous plate osteosynthesis,MIPPO)治疗胫骨平台骨折的临床疗效。方法选取胫骨平台骨折患者59例,按照随机数字表法分为MIPPO治疗组(观察组)和普通切开复位治疗组(对照组),比较两组治疗效果。结果观察组手术切口长度、住院时间、愈合时间等指标均优于对照组,两组比较差异具有统计学意义(P0.05);两组手术时间比较差异无统计学意义(P0.05)。随访2个月、4个月时观察组Rasmussen评分优良率高于对照组,两组比较差异具有统计学意义(P0.05);随访6个月两组间比较差异无统计学意义(P0.05)。结论关节镜联合MIPPO能减小胫骨平台骨折患者手术创面,具有微创治疗的优点,还可以缩短患者住院时间、加速患者骨折的愈合,有助于膝关节功能的恢复,具有较好的临床应用价值。

Analysis on the efficacy of arthroscopy in conjunction with minimally invasive percutaneous plate osteosynthesis in the treatment of tibial plateau fracture

Objective To investigate the efficacy of arthroscopy in conjunction with minimally invasive percutaneous plate osteosynthesis （MIPPO） in the treatment of tibial plateau fracture. Methods Fifty-nine cases of patients with tibial plateau fractures were randomly divided into the MIPPO treatment group （observation group） or the conventional open reduction treatment group （control group）, and the efficacy were compared. Results The incision length, length of stay, and healing time of the observation group were better than those of the control group（All P＜0.05）. There was no statistically significant difference in the operative time between the two groups （P＞0.05）. The Rasmussen rated excellent and good rate of the observation group of 2 and 4 months′ follow-up were both higher than those of the control group（Both P＜0.05）, while there was no statistically significant difference at six months′ follow-up（P＞0.05）. Conclusion Arthroscopy in conjunction with MIPPO for the tibial plateau fractures can reduce the surgical wound, which shows the advantage of minimally invasive treatment. In addition, it can shorten the length of hospital stay, accelerate the fracture healing, and help restore knee function, implying its excellent clinical value.
