L-精氨酸对婴幼儿室间隔缺损修补术心肌缺血/再灌注损伤的影响

目的探讨L-精氨酸（L-Arg）加入心脏停搏液对婴幼儿心肌缺血/再灌注的影响。方法择期行室间隔缺损（VSD）修补术患儿18例,随机分为对照（C组）和L-Arg组（L组）各9例,L组停搏液中加入L-Arg5g/L（24mmol/L）,C组停搏液中加入相同容积9g/L盐水。分别于诱导后（基础值,T0）、主动脉开放30min（T1）、停体外循环（CPB）6h（T2）、停CPB24h（T3）抽取动脉血,测血浆心肌肌钙蛋白I（cTnI）、丙二醛（MDA）、超氧化物歧化酶（SOD）水平,记录观察指标。结果二组血浆cTnI、MDA水平及SOD活性同时点比较差异无统计学意义（P〉0.05）。二组观察指标差异无统计学意义（P〉0.05）。结论含5g/LL-Arg的心脏停搏液对婴幼儿心肌缺血/再灌注损伤无影响。

Effects of L-Arginine on Infants Myocardial Ischemia-Reperfusion Injury during Repair of Ventricular Septal Defect

Objective To investigate the effects of L-arginine(L-Arg)cardioplegic solution on infants myocardial ischemia-reperfusion.Methods Eighteen patients scheduled for repair of ventricular septal defect(VSD)were randomly divided into 2 groups:L-Arg group(L group,n=9)received L-Arg 5 g/L in cardioplegic solution,control group(C group,n=9)received saline solution in the same fashion.Blood samples were taken from artery after induction(T0),30 min after opening aorta(T1),6 h after termination of CPB(T2),24 h after termination of CPB(T3)for determination of the level of cTnI,MDA and SOD.Clinical parameters were observed and compared.Results The level of cTnI,MDA and SOD of 2 groups had no significant difference at the same time(P>0.05).Clinical parameters have no significant difference(P>0.05).Conclusion 5 g/L L-Arg cardioplegic solution has no effect on infants myocardial ischemia-reperfusion injury.
