老年人迟发性癫痫发作的临床及脑电图分析

目的 :探讨老年人迟发性癫痫发作的临床及脑电图特点。方法 :回顾性分析 90例老年人迟发性癫痫的临床及脑电图资料。结果 :80例有明确病因 ,分别为脑血管病 4 6例 (脑梗死 34例、脑出血 12例 ) ,脑肿瘤 2 1例 ,脑外伤4例 ,脑萎缩 9例。临床发作类型为部分性发作 6 2例 (单纯部分性发作 4 0例 ,复杂部分性发作 2 2例 ) ,全面性发作 2 3例 ,不能归类的发作 5例。脑电图正常 8例 ,异常 82例。异常脑电图主要表现为弥漫性慢活动 2 5例 ,局限于一侧半球的慢活动 38例 ,散在或阵发性棘波、尖波或棘慢、尖慢综合波 5 5例。结论 :脑血管病 (脑梗死、脑出血 )是老年迟发性癫痫发作的主要原因。临床以部分性发作为主 ,脑电图异常率高 ,主要表现为弥漫性慢活动基础上出现癫痫样放电。

Analysis of EEG and Clinical Manifestations of Old Patients with Late Onset Epileptic Seizures

Objective To explore the characteristics of clinical manifestations and EEGs in old patients with late onset epileptic seizures.Methods Clinical and EEG date of 90 old patients with epilepsy were analyzed retrospectively.Results The cause of disease was determined in 80 patients, including 46 cases of cerebral vascular disease(34 cerebral infarction and 12 cerebral haemmohrage),21 brain tumors,4 brain trauma and 9 brain atrophy.The types of seizures included partial seizures in 62 patients(40 patients with simple partial seizures,and 22 ones with complex partial seizures),generalized seizures in 23 ones, and unclassified seizures in 5 patients. Normal EEGs were found in 8 patients and abnormal ones in 82 patients.The abnormal EEGs mainly showed diffuse slow waves in 25 patients,slow waves localized in one hemisphere in 38, epileptiform discharges in 55 patients. Conclusions Cerebral vascular disease might be the main causes of old patients with late onset epileptic seizrues.Clinical manifestations mainly show partial seizures.EEG changes mainly showed epileptiform discharges on the background of diffuse slow waves.
