| 摘 要： | 目的比较动力髋部螺钉(DHS)和半髋关节置换治疗股骨粗隆间骨折的临床疗效及预后,探讨治疗股骨粗隆间骨折的可靠内固定方法。方法将股骨粗隆间骨折患者60例分为2组:治疗组30例行半髋关节置换治疗,对照组30例行DHS内固定治疗,观察2组关节功能状况、手术时间、术中出血量、住院时间、术后卧床时间、术后并发症发生情况及术后1 a髋关节功能等。结果对照组术后发生肺部感染3例,泌尿系感染1例;内固定失败3例,其中螺钉退出2例,术后骨折不愈合1例。治疗组术后并发肺部感染1例,经积极内科治疗痊愈。治疗组住院时间和术后卧床时间明显短于对照组。术后1 a,治疗组关节功能恢复情况好于对照组。结论半髋关节置换术具有住院时间短、术后卧床时间短、允许术后早期完全负重活动、并发症发生率低、术后功能恢复好等优点,适用于高龄、骨折类型不稳定、合并严重骨质疏松、健康状况差、预期寿命短的患者,是治疗高龄患者粉碎股骨粗隆间骨折的较合理手术方法。
