三腔胃肠管辅助治疗内镜术后消化道穿孔的作用研究

目的探讨三腔胃肠管辅助治疗内镜操作后上消化道穿孔的疗效。方法采用前瞻性对照研究，对112例内镜操作后出现上消化道穿孔的患者进行保守治疗，对实验组（留置三腔胃肠管及肠内应用雷贝拉唑片剂）与对照组（留置传统胃管及静脉使用奥美拉唑针剂）之间腹痛、腹胀的缓解率、胃液pH值、创面愈合时间、住院时间及保守治疗成功率等情况进行比较。结果实验组6h内腹痛缓解率61．3％、12h腹痛缓解率83．2％，均明显高于对照组（P〈0．05）。实验组住院时间、钛夹未夹闭创面病例的创面愈合时间，均明显短于对照组（P〈0．05）。2组的胃液pH值及保守治疗成功率差异无统计学意义（P〉0．05）。结论三腔胃肠管辅助治疗内镜术后消化道穿孔的效果理想。

The curative effect of triple lumen feeding tube on upper gastrointestinal tract perforation after en- doscopy

Objective To explore the curative effect of triple lumen feeding tube on upper gastrointestinal tract perforation after endoscopic operation. Methods A total of 112 patients with upper gastrointestinal perforation after endoscopic operation were involved in this prospective control test. The remission rate of abdominal pain and abdominal distension, gastric pH, lesion healing time, length of stay and the success rate of conservative treatment were compared between the control group （ indwelling traditional gastric tube and intravenous use of omeprazole injection）and the experimental group （intestinal administration of PPI through triple lumen feeding tube）. Results The remission rate of abdominal pain of the experimental group was 61.3% at 6 hours, and 83.2% at 12 h, which were significantly higher than those of the control group （ P 〈 0. 05 ）. The lesion healing time（ of cases where lesions were not clipped） and length of hospitalization of the experimental group were significantly shorter than those of the control group（ P 〈 0. 05 ）. The gastric pH and success rate of conservative treatment of the two groups had no significant difference（ P 〉 0.05 ）. Conclusion The curative effect of triple lumen feeding tube on upper gastrointestinal tract perforation after endoscopic operation is ideal.
