可疑原发性开角型青光眼早期患者后视路磁共振弥散张量成像特征分析

目的探讨可疑原发性开角型青光眼(POAG)早期患者后视路磁共振弥散张量成像(DTI)特征。方法选取2017年3月-2019年3月收治的可疑POAG早期的45例患者,45只眼为可疑POAG,设为可疑组,选取同期检查的45例正常患者,设为对照组。两组患者均检查视神经纤维层(RNFL)、视野、视觉通路。结果可疑组RNFL厚度、视野平均缺损值(MD)指数均显著低于对照组(P<0.05);两组平均敏感度(MS)指数经比较,无显著差异(P>0.05);可疑组视束、视交叉、视放射的部分各向异性分数(FA)值均显著低于对照组(P<0.05);其表观弥散系数(ADC)值与对照组相比,无显著差异(P>0.05);两组外侧膝状体体积经比较,无显著差异(P>0.05)。结论可疑POAG早期患者视功能正常,但通过DTI技术检测,发现视神经FA发生显著变化,其视觉通路结构异常,可作为早期诊断POAG的依据。

Analysis of the Characteristics of MR Diffusion Tensor Imaging of the Posterior Visual Pathway in the Early Patients with Suspected POAG

Objective To investigate the characteristics of DTI of posterior visual pathway in patients with suspected POA in early stage.Methods 45 patients in the early stage of suspected POAG admitted in our hospital from March 2017 to March 2019,45 eyes were suspected POAG,which was set as suspicious group,and 45 normal patients were selected as control group.RNFL,visual field and DTI were examined in both groups.Results RNFL thickness and MD index in the suspicious group were significantly lower(P<0.05).There was no significant difference in MS index between the two groups(P>0.05).The FA values of the suspicious group were significantly lower than those of the control group(P<0.05).There was no significant difference in ADC between the two groups(P>0.05).There was no significant difference in the volume of lateral geniculate body between the two groups(P>0.05).Conclusions The abnormal structure of visual pathway in patients with suspected POAG can be used as the basis for early diagnosis of POAG.
