神经内镜手术联合脑室外引流术与单纯脑室外引流术治疗脑室出血的疗效比较

目的 对比分析神经内镜手术联合脑室外引流术与单纯脑室外引流术治疗脑室出血的疗效。方法 回顾性分析2016年1月至2018年12月收治的62例脑室出血的临床资料。神经内镜手术联合脑室外引流术治疗34例（内镜组），单纯脑室外引流术治疗28例（引流组）。结果 与引流组相比，内镜组血肿清除率明显提高（P<0.05），引流管留置时间和住院时间明显缩短（P<0.05），二次置管引流率明显降低（P<0.05）。两组术后12周脑积水发生率、术后再出血和颅内感染发生率、术后6个月GOS评分均无统计学差异（P>0.05）。结论 神经内镜手术联合脑室外引流术治疗脑室出血，可迅速有效清除脑室内血肿，缩短住院时间。

Comparison of curative effect on intraventricular hemorrhage:neuroendoscopic surgery combined with extraventricular drainage vresus simple extraventricular drainage

Objective To compare the curative effect of neuroendoscopic surgery combined with extraventricular drainage vresus simple extraventricular drainage on the patients with intraventricular hemorrhage(IVH).Methods The clinical data of 62 patients with IVH,of whom 34 patients received neuroendoscopic surgery combined with extraventricular drainage(neuroendoscopic group)and 28 received simple extraventricular drainage(drainage group)from January 2016 to December 2018,were analyzed retrspectively.Results The hematoma removal rate(83.1%±1.71%)in the neuroendoscopic group was significantly higher than that(69.24%±2.41%)in the drainage group(P<0.05).The indwelling time of drainage tube(7.44±0.46)days]and hospital stay(10.89±0.65)days]in the neuroendoscopic group were significantly shorter than those(15.79±0.65)days and(22.93±1.49)days,respectively]in the drainage group(P<0.05).There was no statistical difference in the rates of intracranial infection,rebleeding and hydrocephalus within 12 weeks after the operation as well as the GOS score 6 months after the operation between the two groups(P>0.05).Conclusion For the patients with IVH,neuroendoscopic surgery combined with extraventricular drainage can increase the hematoma removal rate and reduce the hospital stay.
