产前彩色超声在诊断子宫瘢痕部位胎盘植入中的应用

摘    要:目的探讨产前彩色超声在诊断子宫瘢痕部位胎盘植入中的应用价值。方法回顾性分析2018年3月至2019年1月本院收治的疑似子宫瘢痕部位胎盘植入产妇63例临床完整资料,均于产前1 d进行二维超声及彩色超声检查。以术后病理检查结果为金标准,分析二维超声及彩色超声检查在子宫瘢痕部位胎盘植入中的诊断灵敏度、特异度及准确度。结果 63例疑似子宫瘢痕部位胎盘植入,经病理确诊45例为子宫瘢痕部位胎盘植入,18例子宫瘢痕部位胎盘未置入;彩色超声诊断子宫瘢痕部位胎盘植入灵敏度、特异度及准确度均高于二维超声诊断,差异有统计学意义(P<0.05)。结论产前彩色超声诊断子宫瘢痕部位胎盘植入具有较高的诊断效能,可为临床产前诊疗提供更加客观的参考依据。

Abstract:Objective To investigate the application value of prenatal color ultrasound in diagnosis of placenta implantation in uterine scar.Methods The clinical data of 63 pregnant women with suspected placenta implantation in uterine scar who admitted in the hospital from March 2018 to January 2019 were retrospectively analyzed. All selected cases underwent two-dimensional ultrasound and color ultrasound examinations before delivery for 1 d. the postoperative pathological results were taken as the gold standard, the sensitivity, specificity and accuracy of two-dimensional ultrasound and color ultrasound in diagnosis of placenta implantation in uterine scar were analyzed. Results There were 63 cases of suspected placenta implantation in uterine scar, and 45 cases of placenta implantation in uterine scar were confirmed by pathology, and 18 cases had no placenta implantation in uterine scar;The sensitivity, specificity and accuracy of color ultrasound in diagnosis of placenta implantation in uterine scar were higher than those of two-dimensional ultrasound(P<0.05). Conclusions Prenatal color ultrasound in diagnosis of placenta implantation in uterine scar has higher diagnostic efficiency, which can provide objective reference for clinical prenatal diagnosis and treatment.
