彩色多普勒超声在氩氦刀治疗肿瘤中的应用

摘    要:目的观察氩氦刀靶向冷冻治疗肿瘤术中彩色多普勒超声的应用价值。方法氩氦刀靶向冷冻治疗各种恶性肿瘤27例,共31例次。利用彩色多普勒超声检查肿瘤并设计布置冷冻探头,结合穿刺引导架进行引导并实时监控冷冻过程,进行后期复查。所有患者术后当日或次日接受增强CT检查,计算冰球覆盖率。结果 27例中,24例接受一次冷冻治疗,3例患者因肿瘤过大及肿瘤分散接受多次治疗。根据病灶大小确定冷冻探头布置方案。冰球表现为半圆形后伴声影,在前15min冰球迅速增大,15~20min冰球增大不明显,随着肿瘤体积增大而冰球覆盖率逐渐减小。结论氩氦刀靶向冷冻治疗肿瘤中,彩色多普勒超声技术可进行实时监控,具有较高临床应用价值。

Abstract:Objective To explore the value of color Doppler ultrasonography in Argon-Helium cryoablation treatment of tumors.Methods Twenty-seven patients (31 times) of tumors were treated with Argon-Helium cryoablation under the guidance of color Doppler ultrasound and punctuation-guide device.All patients received enhanced CT,and the melted volume was calculated.Results Of all 27 patients,24 received once cryoabation,the other three received at least 3 times because tumors were larger and scattered.The treatment programs were disposed on the basis of the size of the tumors.The ice ball presented a half circle shape being followed by phonic shadow; and became larger rapidly in the first 15 min,whereas in 15—20 min,the development of the ice ball was slow,and the ice ball covered smaller as the size of the tumor grew larger.Conclusion In the argon-helium cryoablation of tumors,color Doppler ultrasound is useful for real time observation,therefore is highly effective in clinic treatments.
