| 摘 要： | 目的:探讨使用强生Ultracision sealpel system Gen04超声刀不分离胆囊床、不解剖胆囊三角的胆囊切除的临床经验和可行性.方法:回顾性分析我院2005-2009年期间27例急性梗阻性结石性胆囊炎患者使用超声刀一期切除胆囊的临床治疗经验.结果:所有患者均获手术成功,术后疼痛即明显缓解,1～3 d后体温、白细胞逐渐恢复至正常.术后引流管放置3～7 d,所有患者未发生感染、出血、胆漏等并发症;随访1～5年,脂肪泻、消化不良等消化道症状均在6个月后逐渐恢复,但未发现结石复发、胆道狭窄、胆囊切除术后综合征(PCS)发生.结论:伴有严重感染的急性梗阻性胆囊炎,使用超声刀不分离胆囊床、不解剖胆囊三角的胆囊切除是安全可靠的有效方法.
