脑梗死急性期血压水平与近期预后关系

目的探讨脑梗死急性期血压水平与近期预后的关系。方法对220例起病48h内应用脱水剂前的急性脑梗死患者随机测量收缩压/舒张压及MRS评分,判断其日常生活能力和近期预后。结果220例脑梗死病例中186(84.55%)例符合JNC-7高血压诊断标准,14(6.36%)例有近期预后差的趋势。单因素分析显示高血压史与高SBP(P<0.01)及高DBP(P<0.001)相关,而且糖尿病史、意识障碍、病情重与近期预后差相关。但多因素分析示均无显著性差异。结论缺血性卒中急性期高收缩压、高舒张压有近期预后差的趋势,但多因素分析示无显著性差异,可能卒中急性期高收缩压、高舒张压不是独立的近期预后因素。

Relationship between blood pressure changes in acute phase of cerebral infarction and its early prognosis

Objective It is to approach the relationship between changes in blood pressure in acute phase of cerebral infarction and its early prognosis.Methods 220 patients admitted within 48 hours of stroke onset without dehydration were selected,SBP/DBP and MRS scores of them were randomly measured and employed to evaluate activities of daily life(ADL) and early prognosis.Results 186(84.55%) cases of 220 patients with cerebral infarction were consistent with the JNC-7 high blood pressure diagnostic standard,and 14(6.36%) cases tended to bad prognosis.Single Factor Analysis showed that the history of hypertension was relation to high SBP(P<0.01) and high DBP(P<0.001),furthermore,the early prognosis were correlation with diabetes,disturbance of consciousness and bad illness.But Multifactor Factor Analysis displayed no significant difference.Conclusion The high SBP and high DBP in acute phase of cerebral infarction will lead to bad prognosis,but Multifactor Factor Analysis displayed no significant difference.It is likely that the high SBP and high DBP in acute phase of cerebral infarction actually is not the solo factor affecting early prognosis.
