摘    要:目的 测定并比较2型糖尿病不同分期视网膜病变患者及正常人泪液中白细胞介素-6( inter-leukin-6,IL-6)的含量,探讨泪液白细胞介素-6与糖尿病及糖尿病视网膜病变的关系.明确泪液IL-6与血清IL-6水平的相关关系.方法 正常对照者(normal control group,NC组)30例、2型糖尿病无视网膜病变患者(NDR组)30例、2型糖尿病单纯性视网膜病变患者(DR组)30例,2型糖尿病增生性视网膜病变患者(PDR 组)30例,4组年龄、性别比例相匹配.用ELISA法检测4组人群泪液、血清中IL-6的含量.实验资料采用SPSS 12.0统计软件分析,数据比较用单因素方差分析,并对泪液、血清IL-6进行相关分析.结果 泪液中IL-6的含量:NC组浓度为(3.12±1.07 )pg/mL,NDR组浓度为(10.23±3.16)pg/mL,DR组浓度为(16.42±5.71)pg/mL,PDR组浓度为(25.13±4.96 )pg/mL.组间两两比较均差异有统计学意义(P<0.01或P<0.05).血清中IL-6的含量:NC组浓度为(88.86±16.71)pg/mL,NDR组浓度为(126.43±20.93)pg/mL,DR组浓度为(240.42±39.21 )pg/mL,PDR组浓度为(269.50±30.20 )pg/mL.组间两两比较差异均有统计学意义(P<0.01).泪液IL-6与血清IL-6水平有相关性(r=0.758,P <0.05).结论 糖尿病患者泪液IL-6含量增高;随着糖尿病视网膜病变程度的加重,泪液IL-6含量逐渐升高,提示泪液IL-6水平与糖尿病及不同分期糖尿病视网膜病变的发生有关.泪液IL-6与血清IL-6水平之间呈直线正相关关系.

Abstract:【Objective】 In the study,we detect the concentration of the interleukin-6(IL-6) in tears of type 2 diabetic patients(including no diabetic retinopathy patients,pure retinopathy patients and proliferative retinopathy patients) and normal human.Through comparing the variation of IL-6 in four groups,we investigate the relationship between type 2 diabetes mellitus,diabetic retinopathy and the tear IL-6.To explore the correlation of tear IL-6 and serum IL-6.【Methods】 Four groups of people were selected: The first group(30 cases) is normal control(NC) group,the second group(30 cases) is no diabetic retinopathy(NDR) group,the third group(30 cases) is pure retinopathy(DR) group and the fourth group(30 cases) is proliferative retinopathy(PDR) group.There is a match of sex and age among three groups.Gathering tears of four groups.Tear samples were collected atraumatically from the conjunctival cul-de-sac of both eyes by using graduated disposable microcapillaries.Care was taken to avoid touching the corneal and conjunctival surface.All tear samples were collected to detect the concentration of IL-6.Microtubes with tear samples were stored at-80°C until assayed.The samples were obtained from right eye and left eye of each individual,and were pooled.After gathered serums of four groups,we used Enzyme-linked immunosorbent assay(ELISA) method to determine the tear IL-6 and serum IL-6.The OD value of IL-6 could be detected in all cases by ELISA,according to standard curve to find the concentration of IL-6.The sensitivity of the IL-6 concentration was >0.7 pg/mL.The data were shown by(x±s).One-way ANOVA was used to compare the differences between four groups.The experimental data was analyzed by SPSS 12.0 statistical software.【Results】 Concentrations of tear IL-6 were(3.12 ± 1.07) pg/mlL in the control group,(10.23 ± 3.16) pg/mL in the no diabetic retinopathy group,(16.42 ± 5.71) pg/mL in the pure retinopathy group and(25.13 ± 4.96) pg/mL in the proliferative retinopathy group.Concentrations of serum IL-6 were(88.86 ± 16.71) pg/mL in the control group,(126.43 ± 20.93) pg/mL in the no diabetic retinopathy group,(240.42 ± 39.21) pg/mL in the pure retinopathy group and(269.50 ± 30.20) pg/mL in the proliferative retinopathy group.The concentration of IL-6 in NDR group was significantly higher than that in NC group(P <0.01).The concentration of IL-6 in DR group was significantly higher than that in NC group(P <0.01).The concentration of IL-6 in PDR group was significantly higher than that in NC group(P <0.01).The concentration of IL-6 in DR group was significantly higher than that in NDR group(P <0.01).The concentration of IL-6 in PDR group was significantly higher than that in NDR group(P <0.01).The concentration of IL-6 in PDR group was significantly higher than that in DR group(P <0.05).The level of tear IL-6 had positive correlation with the level of serum IL-6(r =0.758,P <0.05).【Conclusion】 The levels of tear IL-6 in diabetic patients(including no diabetic retinopathy patients and diabetic retinopathy patients) increase;With the severity of diabetic retinopathy gradually the levels of tear IL-6 increase,which indicate that it is related to the diabetic,especially the occurrence of diabetic retinopathy at different phase.The level of tear IL-6 closely correlates with the level of serum IL-6.
