腔镜下甲状腺良性疾病手术治疗的探讨

目的探讨经胸乳径路腔镜甲状腺良性疾病手术的临床应用价值。方法对24例应用腔镜行甲状腺手术患者的临床资料进行回顾性分析总结。结果23例顺利完成腔镜甲状腺手术，1例术中快速冰冻切片为甲状腺乳突状癌，中转开放手术。手术时间45～230min，平均121min。病理检查结果：甲状腺腺瘤2例，结节性甲状腺肿8例，结节性甲状腺肿囊性变13例，甲状腺乳头状癌1例．术后1例声音嘶哑，经治疗后3个月恢复。术中术后无大出血、抽搐、无皮下感染积液、脂肪液化，颈部美容满意率97．3％，一般2．7％，不满意0。结论经胸乳入路腔镜下行甲状腺手术切口远离颈部，胸壁切口微小，颈部无瘢痕，美容效果极佳，疗效可靠。

Effect of Endoscopic Thyroidectomy Via Breast Areola Approsch

Objective To investigate the curative effect of endoscopic thyroidectomy via breast areola approach. Methods The clinic data of 24 cases with endoscopic thyroidectomy via breast areola approach were summarized. Results The mean operative time was 121 （45 -230） minutes. Pathologic data showed that 2 cases were thyroid adenoma,8 cases were thyroid nodular goiter,13 cases were thyroid nodular cystis degeneration, 1 case was papillary carcinoma indicated by intra-operative frozen section and con- verted to open operation. The temporary hoarseness occurred in one case and was cured spontaneously in 3 months. No intra-opera- tive and postoperative hemorrhage or convulsion or fat-liquefaction. Satisfaction rate of neck beauty was 97.3% ,general satisfaction rate of beauty was 2.7% ,disaffect rate of beauty was 0. Conclusion The endoscopic thyroidectomy via breast areola approach is safe and feasible for patients with thyroid diseases and has excellent cosmetic result.
