摘    要:目的:探讨慢性阻塞性肺疾病(COPD)患者每日1次和2次口服茶碱的血清浓度变化及对24h肺功能的影响。方法:20例严重COPD患者,在治疗前,先检测24h肺功能进行量化评估,然后用双盲交叉设计的方法,随机先后进行茶碱每日2次和1次用法的研究,测定血清茶碱浓度并重复治疗前的研究内容。结果:茶碱20:00服用在次日4:00达峰值,8:00服后在12:00达峰值,谷值均在20:00,在4:00的茶碱浓度每1次高于2次者,在16:00则相反(P〈0.05),茶碱两种方法的治疗对肺功能有相似但明显的改善(P=0.001)。每日1次用法改善症状明显(P=0.0023)。结论:口服茶碱缓解片在清晨有较高的血清浓度较低的血茶碱水平(5~10mg/L),对肺功能就有改善作用,且副作用小。

Abstract:Purpost: To evaluate the effects of theophylline given by twice-daily and oncedaily on 24 hour lung function in severe COPD. Method: Fwenty patients wee assessed. Before, theop hyline given on lung function and polysomnography wer performed, and baseline scores of symptoms were in vestigated. And then, with double blind crossover in design, twicedaily theophylline closing (TD) or once-daily theophylline closing (OD) was given in a randowly assigned seguence, and serum theophylline concentation (STC) was measured, and other puysiological testings were similar to baseline measurements. Result: The peak STC occaved at 4am after administration at 8pm, and that occurred of 12am after administration of 8am, and the trough STC occurred at 8pm in both treatment. STC of OD was higher than that of Tdat 4AM, but TD was higher than OD AT 4PM (P<0.05). There was a similar but signifcant improvement of the lung function after theophylline given for TD or OD (P<0.001).symptoms inproved in both groups, but OD was better (P=0.0023). Conclusion: There was higher STC in early morning in both treatment. The improvement of lung function may well be obtained of lower STC, and the side effects attributed to SHVFUMEI are rare.
