前列腺癌体积的临床预测及其意义

目的　研究以术前前列腺活检资料来推断前列腺癌体积及病理的价值。方法　以 3 3例因前列腺癌而行根治术的患者作为研究对象 ,将术前PSA、PSAD及前列腺 8点活检的结果与前列腺癌体积及病理进行相关分析研究。结果　 (1)前列腺癌体积与术前PSA、PSAD及前列腺活检的结果呈显著正相关 ,而与年龄、术前前列腺体积以及摘除标本的体积无明显相关关系 ;(2 )前列腺活检中阳性点数联合PSA与前列腺癌体积的回归模型预测前列腺癌体积最好 ;(3 )前列腺活检中阳性点数≤ 3点组的癌体积及精囊浸润率低于阳性点数≥ 4点组 ,两者有显著性差异。结论　前列腺 8点活检中阳性点数是预测前列腺癌体积及病理的一个重要参考指标 ,尤其是联合检测PSA ,更增加其预测前列腺癌体积的准确性

Prediction of prostate cancer volume and its clinical significance

Objective To investigate whether or not biopsy parameters are useful to predict the tumor volume and pathological stage in prostate cancer.Methods Thirty-three patients with prostate cancer who had received radical prostatectomy were studied.The correlation of prostate cancer volume and initial PSA,PSAD and biopsy parameters was analyzed as well as the pathological stage.Results (1) The prostatic cancer volume positively correlated to PSA,PSAD and the parameters from systematic biopsy and there is no obvious correlation with patients' age and the prostatic volume.(2) The linear regression model of prostatic cancer volume that combined the number of positive biopsy cores with PSA showed the best prediction of prostatic cancer volume.(3) When patients with three or fewer positive biopsies were compared with those with four or more positive biopsies,significant differences were identified relative to the prostate cancer volume and the infiltration rate of seminal vesicle.Conclusions The number of positive biopsy cores in systematic biopsy is an important prognostic indicator of pathologic stage and prostate cancer volume.The accuracy for prediction of prostate cancer volume can be enhanced when combined with PSA.
