保留喉功能的下咽癌手术

目的探讨保留喉功能下咽癌手术的可行性及相应保留喉功能方法。方法25例下咽癌患者行保留喉功能的手术治疗，根据不同的肿瘤切除方式，分别进行组织缺损的修复，术后均予以辅助放射治疗。结果25例患者均恢复了发音功能，其中喉功能全部恢复（发音、呼吸和吞咽保护）17例，喉功能部分恢复（发音、吞咽保护）8例。3年、5年生存率分别为68.00%（17/25）和44.00%（11/25）。结论保留喉功能的下咽癌手术是可行的，根据具体情况合理选用咽喉功能重建方法是提高患者生存质量的重要保证。

Preserving Laryngeal Function in Surgery of Hypopharyngeal Carcinoma

Objective To investigate the feasibility and the techniques of laryngeal function preservation in surgical treatment of hypopharyngeal carcinoma. Methods 25 cases with hypopharyngeal carcinoma were treated with function-sparing laryngectomy and received postoperative radiotheraphy and the defect of laryngeal pharynx was constructed by various material such as muscular fasciae and so on. Results Vocalization was recovered in all the patients and completing restoration of laryngeal function was achieved in 17 cases. The 3-year survival rate was 68% and 5-year survival rate was 44%. Conclusion It is feasible for major patients with hypopharyngeal carcinoma to preserve the normal parts of the larynx and restitute the function of the larynx. The effect of operation is satisfactory.
