急性胰腺炎的临床治疗探讨

目的 :探讨急性胰腺炎的病因、诱因、诊断方法及处理对策。方法 :对 1995— 2 0 0 1年收治的 5 0例急性胰腺炎患者进行回顾分析。结果 :多数患者发病原因不明 ;诊断依据临床表现、血尿淀粉酶测定及影象学检查 ;急性单纯性胰腺炎以非手术治疗为主 ,重症胰腺炎以手术治疗为主。结论 :急性单纯性胰腺炎以非手术治疗为主 ;重症胰腺炎以手术治疗为主 ,主张延期手术 ,术式应首选彻底清除胰腺坏死组织加通畅引流 ,强调术前术后支持治疗的重要性

Discussion of clinical treatments in acute pancreatitis

Objective: It is to discuss the cause, remote causes, diagnostic methods and treating ways of acute pancreatitis. Methods: Fifty patients with acute pancreatitis from 1995 to 2000 were reviewed and analyzed. Results: The causes of the most patients were not clear. Diagnosis depended mainly upon the clinical manifestations the tests of amylopsin in serum and urine and the examinations with CT or/and B type ultrasound. Medicine treatment was the major method in treating acute simple pancreatitis, and operation treatment was the principal ways to treat acute pancreatitis. Conclusion: Medicine treatments can get satisfactory effect in acute simple pancreatitis. Operation treatment is mainly used in acute necrotic pancreatitis. Delayed operations with thoroughly resection of necrotic pancreatic tissue and with free intraperitoneal drainage are recommended. The supporting treatments before and after operation have more significance.
