Sky膨胀型骨扩张器在骨质疏松性椎体压缩骨折后凸成形中的初步临床应用

摘    要:目的:探讨Sky骨扩张器经皮椎体后凸成形术治疗骨质疏松性椎体压缩骨折的近期临床疗效。方法:对18例24个椎体的骨质疏松性压缩骨折患者,应用Sky骨扩张器进行经皮椎体后凸成形治疗,观察患者术前、术后疼痛视觉模拟评分(VAS),测量手术前后各病椎椎体前缘、后缘高度,进行比较和统计学分析。结果:24个椎体均为单侧经椎弓根穿刺成功完成手术,失血量平均每例约15ml,每个椎体平均手术时间47min,每个椎体注入骨水泥(4.2±0.8)ml,分布均超过中线。2例骨水泥外漏为沿针道返流,但无临床症状。所有患者疼痛缓解,VAS评分术前(8.2±1.6)分,术后3天(3.1±2.3)分;随访5个月~23个月,随访VAS(2.7±2.8)分(P<0.01)。伤椎前缘平均高度术前17.35mm,术后20.05mm(P<0.01);伤椎后缘平均高度木前22.78mm,术后23.49mm(P<0.05)。结论:Sky骨扩张器是经皮脊柱后凸成形术的新型器械,是治疗骨质疏松性椎体压缩骨折的安全、有效的微创治疗方法。

Abstract:Objective The aim Of this study was to determine the efficacy of“sky-bone expander”kyphoplasty in the treatment of osteoporotic vertebrae compresslon fracture.Methods 18 cases(24 Vertbraes)of osteoporotic vertebrae compression fractures underwent percutaneous kyplasty with sky bone expander system.Patients were evaluated with Visual Analogue Scale(VAS).The anterior height,posterior height of the vertebrae body were measured preoperatively and postoperatively All date was send to paired-samples T test.Results 24 vertebraes were successfully experiented the procedure using unipedicle technique.The average bleed Volume was 15 ml.The average operative time was 47 min.An average volume of(4.2±0.8)ml cement was injected per vertebrae bodv.Extracertebral leakageof bone cement were observed two ertebrae body with no symptom.The VAS score ot the patients was(8.2±1.6)points preoperatively,(3.1±2.3)Points at day 3 postoperatelyand(2.7±2.8)points at follow-up(P<0.01).The average anterior height was 17.35 mm preoperatrly,and 20.05 mm postoperately(P<0.01).The average posteior height was 22.78 mm preoperately and 23.49 mm postoperately(P>0.05).Conclusion Sky bone expander system is new instrument for percutaneous kyphoplasty,is effective and secure in minimally invasive treating osteoporotic vertebral body compression fracture.
