3.0T磁共振诊断胰腺神经内分泌肿瘤的价值及影像特点分析

摘    要:目的研究3.0T磁共振诊断胰腺神经内分泌肿瘤(pancreatic neuroendocrine tumors,PNETs)的价值并总结其影像学特点。方法将医院2014年12月至2018年12月期间收治的31例PNETs患者纳为研究对象,其均行手术或经病理检查确诊为PNETs,所有患者术前均行CT检查,其中13例同时行MRI检查,分析MRI在诊断PNETs中的应用价值。结果 31例患者均行手术切除,术中共发现胰腺神经内分泌肿瘤病灶34个,肿瘤直径0.5-12.04cm,病理检查结果提示G1级18个,G2级12个,G3级4个;所有患者均行CT扫描,共检出29个病灶,漏诊5个,G1级病灶16个、G2级病灶10个、G3级病灶4个;13例患者行MRI检查,共检出15个病灶,无漏诊,G1级病灶7个、G2级病灶6个,G3级病灶1个,其中3D VIBE动脉早期、动脉晚期及门静脉期图像能准确显示病灶全貌,提示病灶大小、质地及其与周围结构间的关系,还可有效反映病灶供血特点。结论 MRI在能有效检出PNETs微小病灶,并对其进行定性分析,在筛查PNETs中具有重要意义。

Analysis of Value and Imaging Features of 3.0T Magnetic Resonance Imaging in the Diagnosis of Pancreatic Neuroendocrine Tumors

Abstract:Objective To study the value of 3.0 T magnetic resonance imaging in the diagnosis of pancreatic neuroendocrine tumors(PNETs) and to summarize its imaging features. Methods 31 patients with PNETs admitted to the hospital from December 2014 to December 2018 were included in the study and were diagnosed as PNETs confirmed by surgery or pathological examination. All patients were given CT examination before surgery, and 13 patients were given MRI at the same time. The application value of MRI in the diagnosis of PNETs was analyzed. Results All the 31 patients were given surgical resection, and a total of 34 pancreatic neuroendocrine tumor lesions were found during surgery with tumor diameter of 0.5-12.04 cm, and the pathological examination showed there were 18 lesions in G1 grade, 12 lesions in G2 grade and 4 lesions in G3 grade. All patients were treated with CT scan, and a total of 29 lesions were detected and 5 lesions were missed diagnoses, and there were 16 lesions in G1 grade, 10 lesions in G2 grade and 4 lesions in G3 grade. 13 patients were given MRI, and a total of 15 lesions were detected and there was no missed diagnosis, and there were 7 lesions in G1 grade, 6 lesions in G2 grade and 1 lesion in G3 grade. The arterial early, arterial late and portal venous images of 3 D VIBE accurately showed the whole picture of lesions, suggesting the size and texture of lesions and their relationship with surrounding structures, and they could effectively reflect the blood supply features of lesions. Conclusion MRI can effectively detect small lesions of PNETs and qualitatively analyze them, and it is of great significance in screening PNETs.
